Objective: To investigate operation approach of ETV for obstructive hydrocephalus patients. Related clinical data were objective analyzed and assessed. To understand the best opportunity,indication and merid of ETV compared with shunt. Method: The endoscope third ventriclostomy were perormed in 13 patients with obstructive hydrocephalus from 2000 to 2004, including aqueductus dilatation were perormed for 1 adult paitant and 1 child paitant. Result: The data of following up from 1 to 48 months showed that of 11 hydrocephalus undergoing the endoscopic neurosurgery,9 paitant excellence, including 5 paitant utility. 8 paitant of postoperative recovery is success, including dilatation of aqueductus were perormed for 2 paitant after ETV. 1 paitant with hydrocephalus and sacral hiatus who has severe meningitis accomplished excision of intraspinal tumor +ETV after the infection was basic controlled, but symptom of hydrocephalus no obvious improve. hemorrhage in ventricle was discovered in 1 child paitant, ETV was operated after resorptioning coagulated blood. the paitant of postoperative recovery was good. Sag of the third ventricle bottom was discovered in 1 paitant with hydrocephalus caused by tumor of pineal region, ETV was given up as ETV might caused injury of basal artery, the paitant was operated using by-pass technique after 6 months. The operation of 1 male paitant was stopped as a result from arterial hemorrhage when ETV. No death case. Conclusion: The endoscope third ventriclostomy are safe and effective operations for obstructive hydrocephalus. The fine instrument strict operation indication and skilled technique are necessary for improving the operative efficacy.
