Deqi(å¾—æ°”) is important in acupucture manipulations, but there aredifferent comprehensions about it. In the schoolbook, Deqi is consideredas Qizhi(气至). In most articles, it is translated as the arrival ofqi, and is also known as acu-esthesia and needling sensation. Deqi refersto the reaction of the merdian qi after the insertion of the needle intothe acupoint. It includes the needling sensation of the patient and thesensation beneath the needle of the acupuncturist. According to aboveinterpretation, there are severel questions about Deqi in the clinicat present. In order to be beneficial to the clinic of acupuncture,theauthor hunts after the original meanings and the development of Deqiby studying the representative literature of acupuncture from acientto modern times. There are four major sections in this paper. The first section, Deqiin Huangdi's Classic on medicine, is part and parcel of the dissertation,because Huangdi's Classic on medicine is the headstream of TCM. In thissection, the author dissertates several issues, including "only afterQizhi does the effect appear",Deqi,waiting for qi,keeping the needlingqi,focusing shen(神), etc. The second section focuses on Deqi in Classicon Medical Problems. The third section is Deqi in past dynasties. Thefourth section is the conclusion part. The author make a brief summaryof Deqi and poses the prospect. The paper not only focuses on the problemof Deqi, but also dissertates topics related,such as reinforcing andreducing, pulse diagnosis, the degree of therapy, keeping the needlingqi, needling sensation of the patient, sensation beneath the needle ofthe acupuncturist and sensation transmission along the meridians ,etc. The author give some conclusions as follow: â'ˆDeqi, which is defined by the schoolbook as needling sensation andsensation beneath the needle,doesn't equal to "only after Qizhi doeseffect appear". â'‰"only after Qizhi does the effect appear" reflects the degreeof therapy. It will not get remedial effect, if you stop the manipulationbefore qi es arrival or still manipulate afte the Qizhi. Qizhi can beget by patient and doctor. Doctor can fell the qi arrival from the changeof pulse of patients and the sensation beneath the needle. This isimportant because the pulse and sensation beneath the needle can betterreflect the developing tendency of the disease. That is to say, it'sdoctor's feelings counts that when patient's and doctor's feelingsare different. â'ŠIn the past, people usually mess Deqi and Qizhi up,as the two notalways mean the same thing. Deqi is commonly considered as Qizhi, itclose correlates to reinforcing and reducing manipulations. Nowadays,Deqi includes needling sensation of the patient and sensation beneaththe needle of the acupuncturist, and doctor uses reinforcing andreducing manipulations after Deqi. Thereby, the author separate theacupuncturist into three ingroups:classical ingroup, traditionalingroup and modern ingroup. The classical ingroup takes Huangdi'sClassic on medicine and Classic on Medical Problems as representation,it takes the sensation of qi beneath the needle as the base of reinforcingand reducing, and it doesn't consider the needling sensation of thepatient. DouHanqing(a famous doctor of Jin-Yuan Dynasty) is therepresentation of the traditional ingroup. The traditional ingrouptakes the sunken or tense sensation beneath the needle as the base ofreinforcing and reducing, it also doesn't consider the needlingsensation of the patient. The modern ingroup originates fromChengDan'an(a famous doctor of modern times), and it attachesimportance to not only the needling sensation of the patient, but alsothe sensation beneath the needle of the acupuncturist. The sensationbeneath the needle of the acupuncturist, mainly is the sunken or tensesensation beneath the needle follows the theory of DouHanqing, whichdevelops not too.
