OBJECTIVE To study the relationship of the plasma high sensitive C-reaction protein (hs-CRP), adrenomedullin (ADM), brainnatriuretic peptide (BNP) and matrix metalloproteinases concentrations with severity of heart falure, the left ventricular ejective fraction and the relationship among them in patients with congestive heart failure(CHF). METHED The blood sample was obtained from 84 patients with congestive heart failure and 40 healthy controls.The plasma level of hs-CRP was measured by immunoturbidometry. The plasma levels of ADM and BNP were determined by radioimmunoassay .The plasma levels of MMP-2 and MMP-9 were measured with ELISA.Left ventricular ejective fraction was measured by echocardiography.Results The plasma hs-CRP,ADM,BNP and MMP-2,MMP-9 concentrations were significantly higher in CHF group than those in control group (P<0.01), The plasma hs-CRP, MMP-2, MMP-9 concentrations increased according to the severity of heart failure by NYHA's classification (P<0.01), The plasma ADM,BNP concentrations also increased significantly according to the severity of heart failure in NYHA's class Ⅱ and classⅢ groups(P<0.01), but they were only little higher in class Ⅳgroup than in classⅢ group and no significantly diffrence between two groups (P> 0.05). The plasma level of hs-CRP, ADM, BNP and MMP-2,MMP-9 had significantly negative correlation to LVEF(P<0.01).There were correlations among the level of hs-CRP,ADM,BNPand MMP-2,MMP-9(P<0.05) ,and the correlations among the level of hs -CRP and MMP-2,MMP-9 were more significant than those among the level of hs -CRP and ADM.BNP.Conclusion (1) hs-CRP,ADM,BNP,MMP-2 and MMP-9 are involved in the begining and development of CHF.they increase according to the severity of heart failure. (2)The plasma hs -CRP and MMP-2, MMP-9 concentrations can evalate the left ventricular function better, and Play important role in the diagnosis, the risk of patients with CHF. (3)MMPs can remold the ventricular.
