Objectives: To discuss and evaluate the efficacies of anterolateral decompression, internal fixation with AF and bone autograft in the contralateral lamina through a modified anterolateral approach for thoracolumbar fresh fractures.Methods: 14 cases were treated with the surgical techniques from 2002. We performed the operation with a post-r>t-ian incision. We first implanted pedicle screws, then took anterolaterai decompression through a modified anterolateral approach, and finally completed reposition of bone fractures and bone autograft.Results: All cases took X-rays in a week after operations. The results demonstrated that the posterior prominence angle and anterior height of vertebral body (AVH) were different sigmficantly(paired t-test P < 0.01 between postoperation and preoper3tion. Th? posterior prominence angle and AVH were obviously regained , and the repositions of bone fractures were good. Followed up for 3-9 months, there was no loss of height of the vertebral body. The functional restoration of nerve was satisfactory. Nervous functions were improved 1-2 grade in Bradford grading scale of 12 patients, and were not improved of two patients after operation. The average improvement was 1.3 grades.Conclusions: 1. The operation through the modified anterolateral approach has less injury and less damage to vertebral bodies' stability. The operations can decompress thoroughly and fix with AF so that vertebral body can get good reposition and recover its shape. 2. The operation can reinforce the stability of spine and 'avoid the loss of height of the vertebral body.
