Objective: This study was undertaken to investigate the effects of CD26/DDPⅣcatalytic inhibitor on ischemia/reperfusion injury during the early stage after lung transplantation.Methods: A rat model of orthotopic unilateral lung transplantation (LTx) was used. 72 syngeneic rats were randomly allocated into control group and investigational group, each group of 36. The lungs were flush-perfused and preserved with LPD or LPD+CD26/DDPⅣcatalytic inhibitor (AB192) in control group or investigational group respectively, and LTx was performed after 18 h cold ischemia time. Grafts were analyzed for arterial blood gas, pulmonary compliance, water content and histological examination in 2-h interval for 24 post-operative hours.Results: During the first 24 h after LTx, and compared with the control group, grafts from the LPD+AB192 group exhibited significantly higher arterial PaO2 (P<0.05) and lower PaCO2 (P<0.05); pulmonary compliance in LPD+AB192 group exhibited significantly higher than control group (P<0.05); wet/dry weight ratio of the grafts in LPD+AB192 group were significantly lower than control group (P<0.05); total lung histopathologic injury scores from LPD+AB192 group were significantly lower than control group (P<0.05) .Conclusion: Inhibitor of CD26/DDPⅣenzymatic activity significantly ameliorated ischemia/reperfusion injury during the early stage after lung transplant.
