Objective:To investigate the prevalence and relevant factors of type 2 diabetes (T2DM) with depression in North Han population. To assess the relationship between brain-derived neurotrophic factor (BDNF) level, the BDNF gene polymorphism and depression, for more understanding the etiology of T2DM with depression.Methods:A total of 366 Han people were involved in the study, being divided into 3 groups:normal control group (NC group,70 control subjects, including 35 males and 35 females, average age was 52.6±4.7 years), non-depression DM group (NDDM group,232 patients, including 129 males and 103 females, average age was 53.1±4.9 years), depression DM group (DDM group, 64 patients, including 19 males and 45 females, average age was 54.3±5.7 years). The volunteers in NC group received CES-D and PHQ-9 questionnaire and had normal score. Basing on the CES-D and PHQ-9 questionnaire, the case group was divided into NDDM group and DDM group. The clinical data were gathered. The Genomic DNA was extracted from the peripheral blood by protease K schizolysis, the BDNF-Val66Met polymorphism was determined with polymerase chain reaction-restriction fragment length polymorphism analysis (PCR-RFLP). Serum BDNF level was measured by ELISA.Results:(1)The morbidity rate was 21.6% in the Han population in the study, the rate of males is 12.8% and 30.4% for females. (2)There were significant differences in sex, study experience, complications (P<0.05), and more significant differences in blood sugar level, BMI (p<0.01) between NDDM group and DDM group, but it were similar with age, marriage, disease duration(P>0.05) at two groups. (3)The serum BDNF level in NC, NDDM and DDM groups was 85.29±3.27pg/ml,79.84±5.15pg/ml and 76.67±5.35pg/ml separately. There is significant difference among 3 groups (P<0.05). (4)There was BDNF-Val66Met polymorphism in three groups (Val/Val,Val/Met,Met/Met), there was significant difference in the constituent ratio of genotype and allele (Val, Met) frequency between NC, NDDM, DDM groups (P<0.05), the frequency of allele Met in DDM group was the highest compared to others. (5)The serum BDNF level was significantly correlated with BDNF genotype. The serum BDNF level in Val/Val, Val/Met and Met/Met group was 83.83±3.79pg/ml,79.04±5.19pg/ml,76.59±5.12pg/ml separately. (6) Sex, HbA1c, BUN, Cr, complications numbers, BDNF allele Val/Met variation were independent risk factors, serum BDNF level was the protective factor by Logistic regression analysis.Conclusions:(1)The morbidity rate of T2DM with depression in female was higher than that in male. (2)The prevalence was associated with sex, study experience, HbA1c, BMI, complications (obesity, peripheral nerve changes, angiosclerosis of lower extremity, hypertension, diabetic retinopathy, coronary heart disease, diabetic nephropathy). The prevalence was not related with age, marriage, disease duration. (3)There was BDNF-Val66Me polymorphism in Han population of the study, including 3 kinds of genotypes:Val/Val, Val/Met and Met/Met. (4)BDNF-Val66Me polymorphism may be the heredity predisposing factor of T2DM with depression. The people taking Met/Met homozygote had more liability of depression than those having Val/Val homozygote and Val/Met heterozygote. (5)BDNF-Val66Met polymorphism influenced the serum BDNF level, BDNF level was correlated with T2DM with depression. (6)The risk factors of T2DM with depression were sex, HbA1c, BUN, Cr, complications numbers, BDNF allele Val/Met variation, and the protective factor was the level of serum BDNF...
