| Objective The study was to survey the baseline level and comprehensive analyze the cognitive level and clinical practice of the doctors and nurses in ICU on the septic shock early goal resuscitation in Qingdao top three hospitals’ intensive care units.It was designed to provide a reference for the clinical managers to formulate management training,thence improve the compliance of evidence-based strategies,the ability of the correct understanding and dealing with the happening of septic shock in ICU and management quality of ICU patients with sepsis.Method Using convenience sampling method, a self-designed questionnaire was designed to investigate the baseline level of 253 doctors and nurses in Qingdao top three hospitals’ intensive care units, we formulated interventions based on the findings and organized management training.Then we selected 66 doctors and nurses in one hospital of Qingdao as learners. After three months training,we investigated again. We used SPSS17.0 to analyze; statistical methods including Student’s t test, chi-square test and one-way ANOVA, difference was statistically significant according to P <0.05.Result1.The baseline survey before training showed that the knowledge cognitive level of doctors and nurses were poor, which nurses’ level more worrying.In the cognitive aspect,ICU medical staff lack of knowledge storage, especially in the three aspects of "septic shock overview" and "3h early recovery goals" and "glycemic control".In clinical practice,ICU medical staff compliance according to evidence-based guidelines was low, especially in the three aspects of "the application of 1h intravenous broad-spectrum antibiotics in ICU", "the application of septic shock early warning system" and "the establishment of septic shock early goal resuscitation specification record".The main factors that influenced clinical practice included the lack of knowledge and standardized guidance and training, the lack of systematic observation assessment process sepsis, routine screening and assessment of risk factors and the lack of adequate resources to quickly identify patients with actual or potential sepsis.2.The survey after training showed that the knowledge cognitive level and clinical practice compliance have improved significantly compared with pre-intervention training,the difference was statistically significant(P <0.05).Conclusion Currently ICU medical staff in Qingdao have low knowledge on the management of septic shock early goal resuscitation.Compared with evidence-based strategies, our current practice situation is not optimistic.So effective continuing education is necessary,ICU managers should give enough attention to organize continuing education which combine with the native culture and available resources, to establish a scientific and standardized management model to improve management quality of ICU patients with sepsis. |
