Objective The Hisense computer-assisted surgery system(Hisense CAS)was used to three-dimensional reconstructionfor for thin layer CT image in children liver,digtial model of liver was obtained, To investigate the value of computer-assisted surgery systems(Hisense CAS) for precise surgery in complexity of liver tumor in children.Methods Clinical data of 21 cases with tumors in the middle lobe or giant liver tumors from June 2012 to January 2015 were analyzed. Computer-assisted surgery system(Hisense CAS) was used to three-dimensional reconstruction and surgical planning for thin layer CT image, it can accurately determine the location of the tumor and adjacent relationship with the surrounding vessels,clear surgical feasibility and guide surgery. The tumor pathological type,operation time, intraoperative-blood loss, blood transfusion rate, complications were analyzed in these cases.Results Three-dimensional reconstruction of 21 cases of children through computerassisted surgery system(Hisense CAS) show the liver,tumor,Hepatic vein,Portal,Hepatic artery,adjacent relationship of tumor and vessel traveling clearly. Operation time:90~240min,Average 130 min.The median volumes of intraoperative blood loss was 60 ml, Minimum amount of blood loss was 3ml,with a blood transfusion rate of 42.9%(9/21). 20 patients remove the tumor successfully by surgery.One case of neonatal with huge liver tumor died of postoperative liver failure and DIC,because of severe obstructive jaundice and severe liver dysfunction.Conclusions Three-dimensional reconstruction of digital model through computerassisted surgery system(Hisense CAS) show the relationship of tumor and vessel and the relationship between tumor and biliary tract clearly. The choice of surgical route based on the specific location of the tumor, Virtual liver resection and Make the best surgical program,It can improve the accuracy and safety in complexity hepatectomy.Objective To reconstruct and observe intrahepatic vascular using Computer Assisted Surgical System(Hisense CAS), and build the typing methods of portal vein for children.Methods Thin-layer computed tomography(CT) imaging of 110 cases were reconstructed using Computer Assisted Surgery System, and typing of the portal vein were established on the basis of its direction and distribution.Results Hisense CAS could clearly show the adjacent relationship of portal vein. Portal veins were classified into four types: a(90/110, 81.8%), b(12/110, 10.9%), c(4/110, 3.6%) and d(4/110, 3.6%).Conclusions It is complex and highly varied of portal vein for children. Hisense CAS could clearly show the adjacent relationship of intrahepatic vascular, provide reliable evidences for the classification of portal vein, and improve success rates of complex hepatectomy.
