Objective:To explore the microscopic objective basis and material basis of three kinds of obesity biased Constitution Classification, by using nuclear magnetic resonance (NMR) based metabonomics.Methods:136 obesity cases in accord with mild physical and three types of physical classification criteria were included in this study, and were divided into 4 groups (i.e.35 cases in normal constitution group,34 cases in phlegm constitution group,31 cases in Qi deficiency constitution group, and 36 cases in phlegm and stasis constitution group) according to constitution types. Serum and urine samples of all the cases would be collected.’H-NMR techniques and multivariate statistical analysis would be used to explore the biomarkers of different physical characteristics.Results:By comparing metabolite spectral features of three kinds of biased constitution obesity, with mild physical as control group, the results showed:1. metabolomics technology could be used as an objective basis for determining constitution. Compared with mild physical, phlegm constitution, qi deficiency constitution, and phlegm and stasis constitution presented qualitative change in metabolism pathways; 2.Potential biomarkers of phlegm constitution, qi deficiency constitution, phlegm and stasis constitution were founded, the content change were defined as well. The urine of phlegm constitution contained citrate (δ2.54,2.66-2.70); and the blood of it contained acetone (82.22-2.23),3-hydroxybutyric acid (81.18,1.22), and citrate (82.50-2.56,2.64-2.70). The urine of Qi deficiency constitutioncontained Creatine (83.02, 3.94), trimethylamine oxide (83.26),hippuric acid (87.54-7.82,7.62-7.66,7.82,3.98), 3-hydroxybutyric acid (81.18,1.22), taurine (3.42,3.26), and glycine (83.58); and the blood of it contained N-acetyl glycoproteins (82.03-2.04), glucose (83.20-4.00), and Glu (82.30-2.40). The urine of Phlegm and stasis constitution contained3-hydroxybutyric acid (81.18,1.22), creatinine (83.06,4.06); and the blood of it contained lipoproteins (80.82-0.88,1.25-1.30), Val (80.97-1.05), glutamine (δ2.10-2.16,2.40-2.50), glucose (83.20-4.00), and lactate(81.32-1.34,4.11-4.13). Compared with the contents of urine or blood in mild physical group, the contents of citrate, trimethylamine oxide, glycine, hippuric acid, and taurine in phlegm constitution were lower, while the contents of creatinine, and 3-hydroxybutyric acid in phlegm constitution were higher. The contents of lactic acid, glycine, and glucose in blood of phlegm constitution were lower while the contents of acetone, lipoprotein, and valine were higher. The contents of citric acid, hippuric, and trimethylamine oxide were lower while the contents of creatine, creatinine were higher in urine of Qi deficiency constitution. The contents of in 3-hydroxybutyric acid, lactic acid, glutamic acid, glycine, and citric acid were lower while the contents of nitrogen acetyl glycoproteins, lipoproteins, and glucose were higher in blood of Qi deficiency constitution. The contents of citric acid, taurine, glycine, trimethylamine oxide, creatinine, and hippuric acid were lower while the contents of creatine, and 3-hydroxybutyric acid were higher in urine of phlegm and stasis constitution. The contents of valine,3-hydroxybutyric acid, glutamic acid, glutamine, citric acid, acetone, glucose, and glycine were lower while the contents of lipoprotein is higher in blood of phlegm and stasis constitution. Compared with Qi deficiency constitution, the contents of trioxide trimethylamine, creatinine, hippuric acid, and 3-hydroxybutyric acid were higher, while the contents of creatine, taurine, citric acid, carbohydrate, and glycine were lower in urine of phlegm constitution, the contents of 3-hydroxy butyric acid, lactic acid, glutamic acid, and acetone were higher, while the contents of acetyl glycoproteins nitrogen, glutamine, glucose, and citric acid were lower in blood of phlegm constitution. Compared with phlegm and stasis constitution, the contents of creatinine was higher, while the contents of 3-hydroxybutyric acid, creatine, hippuric acid, and citric acid were lower in urine of phlegm constitution. The contents of lactate, lipoproteins, glycine, and alanine were higher, while the contents of glucose, and acetone were lower in blood of phlegm constitution. Compared with phlegm stasis quality, the contents of creatinine, glycine, creatine, carbohydrate, and taurine were higher, while the contents of citric acid,3-hydroxybutyric acid, trimethylamine oxide, and hippuric were lower in urine of Qi deficiency constitution. The contents of Valerian acid, N-acetyl-glycoprotein, and glucose were higher, while the contents of alanine, lactic acid, glutamic acid, and citric acid were lower in blood of Qi deficiency constitution.3. According to literature review and the relevant databases searching, the differences of physiological functions and metabolic pathways of metabolite were explained. It came to a conclusion that individual variation of glucose metabolism, lipid metabolism, amino acid metabolism, energy metabolism, neurotransmitter function of organs etc. existed in phlegm constitution, qi deficiency constitution, and phlegm and stasis constitution. It provided certain metabolomics basis for the obesity constitution classification.Conclusions:Phlegm constitution, qi deficiency constitution, have phlegm and stasis constitution obese people have individual differences in glucose metabolism, lipid metabolism, amino acid metabolism, energy metabolism and neurotransmitter function of organs and other aspects of change, which provided metabolomics basis for classification of obesity constitution. There were differences between three biased constitution NMR metabolic profile, which could be identified by specific metabolite from metabolomics analysis, and interpret biological nature of syndromes.
