Ischemic stroke is the most common type of stroke disease, it has a complicated physiological and pathological mechanism. In recent years, the concept of neurovascular unit (NVU) has opened up a new direction for the pathogenesis and treatment strategy of cerebral ischemia.Multiple-target treatments,which aims at at structural damages in NVU,offer better results on neural functional recovery. Professor Zhishun Yu combines basic theories with clinical experience and sums up the method of cluster needling of scalp acupuncture which have remarkable curative effect on ischemic stroke.According to his hypothesis, cluster needling of scalp acupuncturehas a effect on wider range of brain tissue. Accordingly, we assume that cluster needling of scalp acupuncturepromotes the recovery of neurovascular unit after ischemic stroke.Neuron is the dominant component of NVU,synapsis is thefunctional part of neuron which transmit neural signals between neurons or other effector cells. Synapsis has important impact on the interflow between components of NVU.Synaptophysin (SYN) is one kind of calcium-binding proteinswhich locates in synaptic vesicle membrane,it is the symbol of synaptic structure and function reconstruction as well as neuron functional recovery. In order to study themechanism for cluster needling of scalp acupuncture in the treatment of ischemic stroke, we design this animal experimental subject.Objective:To observe the effect of cluster needling of scalp acupuncture in MCAO rats on neurological function scoring and expression of synatophysin in ischemic penumbra. To study the mechanism of cluster needing of scalp acupuncture on neural recovery afer cerebral ischemia.Methods:54 male SD rats were randomly divided into cluster needing group(A),model group(B) and sham operation group(C),18 rats in each group.Then,according to time point after cerebral ischemia(1d、7d、14d), each group was randomly reassigned into 3 subgroups,6 rats in each subgroup;Rats in group A and group B were made by middle cerebral artery occlusion(MCAO) model,group C was only made by sham operation.Group A received treatment of scalp cluster needling,no therapy was given to group B and C. Neurological function was evaluated with Bederson score.Brain sections was stained by mmunofluorescence method and obsereved by confocal microscopy. The positive expression areas of synatophysin were counted by biological image-analysis software.Results:1、Neurological function score:In comparision with group B, the neurological function score of group A decreased on both 7d and14d (P<0.05);In group A, the neurological function score after modeling decreased compared with the score before brain collection on both 7d and 14d (P<0.05);In group B, the neurological function score after modeling decreased compared with the score before brain collection on 14d (P<0.05); 2、Positive expression of synatophysin:In comparision with group B, the positive expression areas of synatophysin increased on both 7d and 14d (P<0.05); In group A, the positive expression areas of synatophysin increased progressively (1d、7d、14d).Conclusion:1.The MCAO model cause the damage of neurologic function and the drop of synatophysin;2.Cluster needling of scalp acupuncture can reduce neurological function scoresafter cerebral ischemia,it has positive impact on neural recovery;3.Cluster needling of scalp acupuncture can increase the expressionof synatophysin,it has promoting effect on the process of neurologic recovery after neuron damage;4.Rats have self-healing effect to certain degree after neurologic damage.
