| Purpose:This article summarizes the literature and understanding of modern tinnitus ancient physicians tinnitus etiology, pathogenesis and acupuncture, while the basis of summarizing progress and mechanism of neurotransmitter monoamine Western medicine have on tinnitus, the tinnitus patients by detecting before and after acupuncture treatment difference in peripheral serum central neurotransmitter levels, and explore acupuncture treatment of tinnitus possible mechanism.Method:Survey Location:Beijing University of Chinese Medicine Oriental Hospital ENTSurvey:newly diagnosed patients with tinnitus as the first chief complaintTime:September 2015 to February 2016Outpatient included 40 patients with tinnitus main complaint, take ear Zhou Yifeng, listen to house, hearing,1.2-1.3 inch vertical needle, local tingling sense of pain for the degree, pintail connection G68050-2A treatment, continuous wave pulse repetition frequency 30Hz, each time the needle for 15 minutes, with points dialectical acupoints, once a day,14 times as a course of treatment; and after application of quadrupole time of flight mass spectrometry (UPLC-Q-Tof/MS) was used to detect patients peripheral serum central neurotransmitter levels.Result:① selected cases of patients with tinnitus total number of cases was 40 cases, including 17 males (42.5%),23 females (57.5%), male to female ratio was 1:1.35; 40 cases of patients in selected cases, the oldest 78 years old, the smallest 33 years, mean 55 years. After normality test, in line with the normal distribution (P=0.2> 0.05). Duration of a minimum of 3 months and a maximum of 10 years. ② After a course of treatment, tinnitus disappeared in 2 cases,14 cases markedly effective in 9 cases,15 cases ineffective, the total efficiency of 62.5%, consistent with the literature, the exact effect, doctors and high patient satisfaction. Increased significantly (P<0.01).③ treatment GABA content; 5-HT content significantly decreased (P <0.01).Conclusion:Through this observation to verify the effectiveness of acupuncture treatment of tinnitus. The establishment of two kinds of peripheral blood monoamine neurotransmitters GABA and 5-HT in UPLC-Q-Tof/MS analysis method for simultaneous determination of sample requirements, simple to operate. The establishment of the method of acupuncture treatment of tinnitus associated monoamine neurotransmitter assay provides a reliable methodology to make peripheral neurotransmitter content can be accurately measured. Increase GABA in the case of the occurrence of tinnitus inhibitory neurotransmitter, GABA determination based on the results of the analysis, the electro-acupuncture treatment of tinnitus possible mechanism to upregulate the expression of the inhibitory neurotransmitter GABA in nerve inhibitory neurotransmitter content so that the sensitivity of the auditory system recovery, reduce tinnitus perception than the front, and then tinnitus symptoms improved. Moreover, from the experimental results that significantly reduced after acupuncture treatment of tinnitus patients blood levels of 5-HT (P<0.01), namely acupuncture treatment to reduce the content of 5-HT, 5-HT pathway passing through damage hearing filtration and practice clothes functional recovery of brain plasticity changes occur so that the central auditory cortex hyperactivity under control, then reduce the symptoms of tinnitus.By detecting changes in blood before and after treatment in two kinds of GABA and 5-HT neurotransmitter content, we can conclude that tinnitus acupuncture treatment may be associated with the modulation by neurotransmitters play a role. |
