Medical image rebuilding in three-dimension is one of the hottest research direction in medical simulation nowadays and the mesh generation is the key technology in the three-dimension rebuilding. Medical image rebuilding is the basis of the whole surgery simulation system. There are a lot of restrictions and demands on the topological construction because of some typical operation implementation such as Finite Element Method, cutting and distortion. This paper combines the traditional simulation method of Marching Tetrahedral and the classical algorithm called Delaunay together and got a new method which can contribute to the medical geometrical construction. This improved MT algorithm use standard tetrahedron as the initialized mesh unit instead of the tetrahedron dividing from the hexahedron. It is obvious that both the quality of the inside mesh unit and the quality of the fringe mesh unit have significant improvement. The inside mesh units have consistent size and ideal quality for the Finite Element Method. This method can establish good foundation for the further steps in virtual surgery system.A reformative algorithm basis on the Delaunay Refinement method to the low quality units in the fringe is introduced in this paper. This method has high efficiency and good adaptability with kind human-machine interface and high stability. This paper stresses the investigation of the basic algorithm of the virtual surgery topological construction and has embedded explanation of the two dimension Delaunay algorithm including the data structure and the experiment results.
