Efects Of Ulinastatin Pretreatment On Serum Hmgb1, Tnf, IL-8 Concentration Changes With One-lung Ventilation Patients

Objective: Observation of serum HMGB1, TNF-α, IL-8 concentration changes with one-lung ventilation patients by pretreatment ulinastatin.Methods: Thirty patients with lobectomy with one-lung ventilation were randomly divided into two groups by pretreated with ulinastatin(group U, n=15) and the group not pretreated with ulinastatin(group C, n=15). All patients were induced with conventional anesthesia,maintained wifh combined inheal and intravenous anesthesia. Then double-lumen tube intubated, and controled ventilation. The patients in the group U were used with ulinastatin at 10000 U/kg, diluted to 100mL and drip completed in 10min, and used saline in group C. Blood-sample were drawn after induction(T0),60 min after one-lung ventilation(T1), at the end of operation with two lung ventilation 120min(T2), and 24h after surgery (T3). Serun HMGB1, TNF-αand IL-8 were assayed with enzyme-linked immunosorbent assay(ELISA). Results: Compared with T0, HMGB1 level was no significant differences at T1(p>0.05), but it were significant lower at T2 and T3(p<0.05). There were no significant differences in TNF-αand IL-8 levels at T0 in both groups. Compared with T0, in each group HMGB1, TNF-αand IL-8 levels were significant higher at T1, T2 and T3(p<0.05). Compared with group C, HMGB1 level was no significant differences at T1 (p> 0.05), but it were lower at T2 and T3(p<0.05) of the group U. Compared with group C, TNF-αand IL-8 levels were significant lower at T1, T2 and T3(p<0.05) of the group U. The postoperative control ventilation time, ICU stay time, and the postoperative complications such as hypoxemia, pulmonary infection, respiratory failure case of the group U, were lower than group C, but the difference was not statistically significant (p> 0.05).Conclusion: Ulinastatin pretreatment may decrease serum HMGB1, TNF-αand IL-8 levels with one-long ventilation patients, and might be related to mitigate of the post operative pulmonary injury.
