Objective: YST is the most common tumor in children's primary malignant GCT. The diagnosis of YST rely on clinical manifestation,radiological information,serum AFP detection, etc, pathological diagnosis is needed to make a definite diagnosis. Using immunohistochemistry can improve the diagnostic accuracy. This experiment is aimed to study the expression of SALL4 in YST of children, and analysis the clinical significance.Methods: The pathological specimens was from the children's hospital of chongqing medical university, 21 cases (11 male, 10 female)were selected, which containing 8 punctura biopsy specimens. Age distribution: 4 months- 3 years old, the average age: 18 months. All cases were diagnosed as YST by pathologists. In addition, 10 MT,5 IMT,1dysgerminoma were retrieved for this study. The expression of SALL4 in YST,MT,IMT and dysgerminoma were detected by SP immunohistochemical method, AFP was also detected in YST to compared with SALL4.Results: (1)The expression of SALL4 was present in all 21 cases of YST(positive incidence:100%), and each case had high intensity of positive expression. SALL4 could be detected positive in each kind of tissue structure in YST(such as: loose reticular network,mamillae,S-D bodies,glandular structure,solid nest). But in the other common children GCT, no expression was present. (2) SALL4 had high intensity of positive expression in children YST, 6 cases had( ++), 15 cases had( +++), but the expression of AFP in YST was relatively weak, 2 cases had (-), 5 cases had (+), 9 cases had (++), 5 cases had (++), SALL4 had higher intensity expression than AFP in YST (P < 0.01);There was no obviously correlation in the intensity of positive between SALL and AFP(p>0.05). (3)In the 8 punctura biopsy specimens, SALL4 was stained (++) in 3 cases, and(+++) in 5 cases; the staining of AFP was 1 (-),2 (+),4 (++),1(+++). the intensity of positive was significant different (P < 0.01).Conclusion: (1)In the common children GCT,SALL4 could become a new relatively sensitive tumor marker for YST. (2)In immunohistochemical study analysis, SALL4 had higher intensity expression than AFP in YST. There was no obviously correlation in the intensity of positive between SALL and AFP. (3)Immunohistochemical staining of SALL4 could be helpful for the diagnosis of children's YST, especially in the punctura biopsy specimens which had less quantity of tissue. SALL4, a more sensitive tumor marker, could be valuable for the the pathologic molecular diagnosis.
