Cleft lip and palate (CLP) are the most common congenital maxillofacial deformities. It not only affects the facial appearance and aesthetics of patients, but also affects oral functions like eating, speaking, and so on. CLP patients have a great chance to show a variety of maxillofacial deformities when they grown up as adults, such as maxilla retrusion, deficiency of the middle 1 / 3 of the face, and anti-bite, ect. It brings a huge emotional and psychological trauma to the patients and their families. The causes of the deformity have not been determined. Orthognathic treatments of patients with cleft lip and palate are more difficult than that of patients with common malocclusion, so the orthognathic surgery of CLP patients has become one of the most challenging directions in this area. How to define the pathological changes of the craniofacial structure, so as to make the preoperative design and outcome prediction accurately are important perspectives of study. Delaire established an architectural balance theory of craniofacial area. The cephalometric analysis based on it is comprehensive, accurate and intuitive, so it is very suitable for the preoperative design and outcome prediction of the orthognathic surgery.The aim of this study was to summarize the characteristics of craniofacial morphology of adult CLP patients, find out the anomalies, and to provide references in revealing the causes of abnormal morphology. Meanwhile discussed the influences of early surgery on craniofacial growth and development.70 adult patients with cleft lip and palate were involved in this study. We analyzed their cephalograms with Delaire architectural analysis, summarized the craniofacial morphology characteristics, compared them with that of the normal occlusal population; Then divided these 70 patients into 3 groups by the time they underwent surgery: group A, B and C. Then compared each group with the normal occlusal population, and made comparison between groups.Results and conclusions are as follows: 1, Adult CLP patients had a maxilla deficiency in sagittal and vertical direction, and the volume of maxilla was reduced; The center of mandible moved upward and forward correspondingly; The front teeth were more erect; Morphology of the skull base changed, which represented the growth and development were blocked; 2,There were little differences of craniofacial morphology between patients who did not receive early surgical repair and the normal occlusion population. Patients who received early surgery showed obvious abnormalities in craniofacial area; 3, The CLP patients themselves have a tendency to show abnormalities in the craniofacial area, and the secondary deformities caused by early surgery is more serious, but the extent of abnormal was within a certain range; 4, There was no big differences between the influences of lip repair surgery only and lip repair plus palate repair surgery on the development of craniofacial area. 5, The craniofacial morphology features of CLP patients under Delaire cephalometric analysis have been summarized.
