Comparion Study On The Dietary Therapy's Affiection Of Infusion Pump For ICU Patients With Different Nasogastric Feeding Durative Time

Objective:This study was designed to contrast three experimental groups, which is different nasal feeding time of infusion pump everyday , that is continuous 24-hours pump group, continuous 16-hour pump group, and intermittent 24-hour pump group, to identify the best continuous time of nasal feeding which is suitable for ICU patients to use in ICU care. In order to lower hospitalization costs and the time of hospital stay, improve nurse quality. Methods: 180 ICU patients were randomly assigned into three groups, each group has 60 cases. The patients who in the continuous 24-hour infusion group and the continuous infusion group received continuous infusion pump nurse, the intermittent 24-hour pump group pump with eight time units, and each time unit is 3h, which continued to pump for 2h and interval for 1h. Three groups carry out randomized controlled clinical trial study, after the nurse intervention, to observe the main index, such as complication and nutritional status, and secondary indicators, such as immunity and the change of patient's condition. Results:①The gastric retention rate of the continuous 24-hour pump group and the continuous 16-hour pump group is significantly higher than that of the intermittent 24-hour pump group, there are differences among the three groups (p<0.05). The pairwise comparison among the three groups, to compare intermittent 24-hour pump group with continuous 24-hour group and continuous 16-hour pump group, p values are less than 0.017,there is statistically significant between continuous 16-hour group and continuous 24-hour pump group,p>0.017; the incidence of diarrhea among the three groups is not statistically significant (p>0.05); the incidence of constipation among the three groups is no statistical significance (p>0.05). Potassium ions in the three groups before and after nasal feeding had no statistically significant (p>0.05), there are differences between the first day and the ninth day in continuous 24-hour pump group (p<0.05). There is no significant in the concentration of sodium ions among the three groups (p>0.05), and the concentration of sodium ions between the first day and the ninth day is not statistically significant (p>0.05);②The serum total protein and serum albumin value of three groups is not statistically significant (p>0.05), but there is significant difference between the first day and the ninth day (p<0.01);③The hemoglobin index of the first day and the ninth day is statistically significant (p<0.05). There is different among Continuous 24-hour pump group, continuous 16-hour pump group, intermittent 24-hour pump group (p<0.05), the compare of continuous 24-hour pump group and the intermittent 24-hour pump group is statistical significance (p<0.017);④The lymph node count of three groups is no statistical significance (p>0.05),the lymph node count is also no statistical significance between the first nasal feeding day and the ninth day (p>0.05);⑤APACHE-Ⅱscore of three groups'patients was no statistical significance, but APACHE-Ⅱscore was statistically significant between the first hospitalization day and the ninth day (p<0.01). Conclusion:①The three kinds of nasal feeding methods play the same role in prevention of complications basically, the intermittent 24-hour infusion method is slightly better than the other two methods in the prevention of gastric retention, and the continuous 24-hour infusion method is slightly worse than the other two methods in the maintenance of potassium ion concentration;②Three kinds of nasal feeding methods play the same role in the maintenance of nutrition,but the nutritional indices of three groups is lower, suggesting that enteral nutrition should be begin as soon as possible. The intermittent 24-hour pump method is worse than the continuous 24-hour pump method in the maintenance of hemoglobin concentration;③Three kinds of nasal feeding methods are able to maintain ICU patients'immunity;④Three kinds of nasal feeding methods are able to improve ICU patient's condition;⑤It can assure ICU patient's rest sufficiently by the continuous 16-hour pump method.
