Clinical Characteristics And Immune Condition Of Lymphocytic Thyroiditis And Papillary Thyroid Carcinoma

Objective: To investigate the meaning of IL-4,IFN-γand analyse image characteristics on CT and Ultrasonography B-mode,thyroid functions of lymphocytic thyroiditis with thyroid papillary carcinoma.Methods : 116 cases of LT,177 cases of PTC and 68 cases of LT with PTC from January 2007 to December 2008 in Tumor Hospital Affiliated to Xingjiang Medical University were analysed, including, age,sex, nation, lymph-node metastases,the image characteristics on CT and Ultrasonography B-mode, thyroid functions.Meanwhile,IL-4,IFN-γwere detected in 32cases of LT,23 cases of PTC ,39cases of LT with PTC.Results:(1)The incidence of LT with PTC was 37%in patients with LT and 27%in patients with PTC. (2)The estimated value of TG,A-TPO was higher by comparing LT with PTC to LT(P<0.05).The estimated value of TG was decreased and A-TPO was increased in the LT with PTC than PTC(P<0.05). (3) For Ultrasonogaphy B-mode and CT,comparing with LT, LT with PTC showed irregular shape, ill-defined margin(P<0.05). For CT findings, comparing with PTC, LT with PTC showed ill-defined margin(P<0.05).(4)For lymphocyte, the expression of IL-4 in LT was higher than that in the LT with PTC(P<0.05);The expression of IL-4, IFN-γwas higher in LT with PTC than that in the PTC(P<0.05).(5)The expression of IL-4, IFN-γin thyrocytes of LT was higher than that in the LT with PTC specimens(P < 0.05).(6)The expression of IL-4, IFN-γin cancerational thyrocytes was significantly higher than that in inflammatory thyrocytes in LT with PTC(P<0.05).(7)Expression of IL-4 in the lymph-node metastases group of the LT with PTC was higher than that without lymph-node metastases group(P< 0.05).Conclusion:(1) There was a high incidence of coexistent LT with PTC. (2) When estimated value of TG,A-TPO in LT were no longer high, it is helpful to find the cases of carcinogenesis. (3) When LT showed irregular shape,ill-defined margin in CT or Ultrasonography B-mode examination ,we should be caution of thyroid carcinogenesis.(4) During the course of carcinogenesis, IL-4 produced by lymphocyte decreased gradually. (5)The expression of IL-4 in LT with PTC may showed the possibility of lymph-node metastases.
