Objective: Majority of spontaneous rupture of spontaneous rupture of primary liver cancer is a serious complication of liver cancer, the occurrence rate of 9% to 22.6%, compared with acute disease, dangerous disease, but if not timely diagnosis and treatment, mortality is high, so rupture of primary liver cancer early detection and prompt and appropriate treatment improve the prognosis is very important. Liver resection is the spontaneous rupture of the most effective treatment method. Emergency liver resection has been advocated in the past, through a surgery to stop bleeding and remove the tumor therapeutic purposes. Emergency liver resection, but has a high surgical risk and hospital mortality, surgical indications and the need to strictly control the timing. Many scholars believe that those who underwent emergency surgery is not easy cases feasible TAE, hepatic artery ligation or gauze packing to stop bleeding and other treatment, when bleeding stopped and the general condition is getting better qualified two possible cases surgery. This paper summarizes the spontaneous rupture of primary liver cancer surgery patients and to analyze spontaneous rupture of hepatocellular carcinoma summary of surgical therapy, to explore the spontaneous rupture of hepatocellular carcinoma suitable surgical treatment.Methods: Retrospectively during the period 2004 to 2007, the First Affiliated Hospital of Nanchang University in surgical treatment of primary liver cancer. 50 cases of spontaneous rupture of primary liver cancer surgery cases and 28 cases were randomly selected from the same period in non-surgical treatment of ruptured liver cancer cases, according to timing of surgery were divided into emergency surgery group, two surgical groups, and to the same period Non-surgical treatment of ruptured liver cancer patients as controls. According to records of the three preoperative patients with general information, medical history, symptoms and laboratory examinations features information and referral or follow-up data after surgery, the three groups in accordance with the complications, survival rates, etc. compared to study the therapeutic effect of three cases.Results: Three groups in gender, age, AFP and liver function classification was no significant difference (P> 0.05). Three groups of patients with liver cirrhosis, tumor size, tumor location was no significant difference (P> 0.05). Three groups of patients no significant difference in operative time (P> 0.05). Blood loss and blood transfusion in Group B and Group C is greater than the difference was statistically significant (P <0.05). Group several complications after surgery were significantly more than B and C. Survival rate after six months, one year survival rate, 3-year survival rate, compared three groups of 22 were no significant differences (P> 0.05).Conclusion: 1, spontaneous rupture of primary liver cancer resection as long as the strict control of emergency indication, to avoid the risk of emergency surgical resection, still the first choice for treatment of liver rupture. 2, spontaneous rupture of hepatocellular carcinoma underwent surgical treatment is almost equivalent to non-ruptured liver cancer, and therefore not suitable for emergency surgery or emergency removal of the more difficult cases can first use of hepatic artery embolization or other methods to stop bleeding treatment, and as far as possible to create conditions for the short term for two liver cancer.
