ObjectiveComparison of wrist blood pressure measuring non-invasive blood pressure and aortic root of the invasive blood pressure. Investigate the accuracy of Wrist Blood Pressure Monitor blood pressure measurement and its influencing factors..MethodsChoose 282 patients who required to accept Coronary Angiography. After puncture successfully in the artery coronary measure the invasive / non-invasive blood pressure three times, taking the average, To adjust the pressure of the transducer, the pressure is zero set. After arterial puncture, 6F catheter will be sent to aortic root. Lead2000 multichannel physiologic recorder tracings pressure curve before coronary angiography,take the paper speed 25mm/sec, aortic systolic pressure and aortic diastolic blood pressure obtained from the curve . non-invasive blood pressure through the wrist were measured at the same time, two or more experts to judge and record the contrast results. According to Coronary Angiography results and hypertension,the patients were divided into 86 cases of coronary heart disease and hypertension, 88 cases of coronary heart disease alone, 57 cases of hypertension were, 51 cases of normal control group, except for peripheral vascular disease combined, Secondary hypertension, diabetes. Take the wrist blood pressure and central aortic pressure through the Group consistent evaluation of the correlation coefficient;Take the difference between wrist blood pressure and aortic systolic pressure and aortic diastolic pressure as index, use two factors-line analysis of variance analysis to evaluate hypertension and coronary heart disease. 282 cases of coronary angiography were divided into 174 cases of CHD, 108 patients of non-CAD group. Take the difference between wrist blood pressure/aortic systolic pressure and aortic diastolic blood pressure as an index,measure the accuracy of wrist blood pressure monitor under the UK standard equipment (BHS).Results1. Central aortic invasive blood pressure and wrist non-invasive blood pressure was significantly related. Coronary heart disease of 86 patients with hypertension invasive SBP (150.75±14.59), DBP (86.92±8.87) (mmHg) and wrist blood pressure measurement non-invasive SBP (155.71±12.56), DBP (87.82±7.52) (mmHg) was positively correlated, the correlation coefficient is 0.758,0.829(P <0.01); 88 patients with coronary heart disease alone invasive SBP (118.04±11.95), DBP (71.95±8.65) (mmHg) and wrist blood pressure non-invasive SBP (126.17±10.23), DBP (72.48±8.42) (mmHg) was positively correlated, the correlation coefficient was 0.695,0.865(P <0.01); 57 patients with hypertension invasive SBP (150.83±9.25), DBP (91.28±9.17) (mmHg) and wrist blood pressure measuring non-invasive SBP (151.50±9.32), DBP (91.58±8.57) (mmHg) was positively related, the correlation coefficient was 0.933,0.915(P <0.01); normal control group, 51 patients invasive SBP (127.97±8.05), DBP (79.47±6.32) (mmHg) and wrist blood pressure measuring non-invasive SBP ( 128.27±7.74), DBP (80.00±5.26) (mmHg) was positively correlated, the correlation coefficient was 0.898,0.81.2. In conclude wrist blood pressure was higher than the center of arterial blood pressure. Coronary heart disease in patients with hypertension group: the difference between aortic blood pressure and wrist non-invasive blood pressure was SBP (4.95±9.48), DBP (0.87±4.80) (mmHg); CHD group: the difference between aortic blood pressure and wrist non-invasive blood pressure was SBP (8.15±8.69), DBP (0.51±4.44) (mmHg); Hypertension group: the difference between aortic blood pressure and wrist non-invasive blood pressure was SBP (0.67±3.40), DBP (0.32±3.66) (mmHg); normal control group, the difference between aortic blood pressure and wrist non-invasive blood pressure was SBP (0.27±3.58), DBP (0.57±3.56) (mmHg).3. Coronary heart disease's influence on the difference of wrist blood pressure and aortic systolic pressure measurement was statistically significant (P <0.001), the difference of diastolic blood pressure measurements was not significant (P> 0.05); Hypertension has no influence on the difference of wrist blood pressure and aortic systolic/ diastolic pressure (P> 0.05)The interaction of two factors on the wrist blood pressure and aortic systolic pressure was not significant (P> 0.05), diastolic blood pressure measurements was no significant difference (P> 0.05).4. Wrist Blood Pressure Monitor Measurement of systolic blood pressure in patients with coronary heart disease was of D-class , the rest are A-level. The difference of Wrist blood pressure measurements and central aortic pressure measurements, according to British equipment rating standards (BHS) grade, 174 cases of CHD patients, the difference in -5 -5mmHg : 65 cases(37.4%), the difference in -10 -10mmHg:93 cases(53.4%) , the difference in -15-15mmHg :150 cases(86.2%), BHS was Class D; diastolic blood pressure difference between -5 -5mmHg:148 cases(85.1%), difference in -10 -10mmHg:172 cases(98.9%), the difference in -15 -15mmHg:173 patients(99.4%), BHS was A-level; Non-CAD group 108 patients, The difference of systolic blood pressure in -5 -5mmHg:91(84.3%), the difference in -10 -10mmHg: 108 cases(100%), the difference in -15-15mmHg:108 cases, the ratio was 100%, BHS was A-level; diastolic blood pressure difference between -5-5mmHg: 89 cases(82.4%), the difference in -10-10mmHg:108 cases(100%); the difference in -15-15mmHg:108 cases, the ratio was 100%, BHS was A-level. Conclusion1.Omron wrist blood pressure and invasive blood pressure were significantly correlated.2.Omron Wrist Blood Pressure Monitor Measurement of systolic blood pressure in patients with coronary heart disease have a certain bias.
