Detection Of Ct And Relationship Between Infection With Ct And Sequela Of Pid

Objective: To investigate the effect of cell culture, polymerase chain reaction(PCR) and Colloidal Gold in diagnosis of Chlamydia Trachomatis (CT) from cervical swabs. To understand the prevalence of CT infection in sequela of PID.To discuss the relation between infection with CT and sequela of PID.Methods: 578 cases of cervical swabs were collected from patients visiting the gynecology clinic of the First Affiliated Hospital of University of South China in August 2008 -June 2009.All of ages ranged from 20 y to 50 y, the average age was 37.3. 300 cervical swabs were from asymptomtic cases and the CT was detected by cell culture, polymerase chain reaction(PCR) and Colloidal Gold. The sample from which cell culture is positive or other two methods are positive, defined as true positive or "expanding gold standard". The others consisted of experimental group and control group. Experimental group were 212 sequela of PID,including 84 infertility,60 ectopic pregnancy and 68 chronic pelvic pain and recurrent PID. Control group were randomly selected early pregnancy flow women 66 over the same time. CT infection was detected by Colloidal Gold in the two groups The chlamydia positivity were analyzed in different age groups according to Colloidal Gold.Results:According to the "expanding gold standard" , 36 cases were diagnosed with CT infection in 300 asymptomtic cases. The chlamydia positivity was 12 %(36/300). There were 6 positive samples and 256 negative samples by three different methods.For cell culture, PCR and Colloidal Gold methods, the sensitivity was 72. 22 %,91. 67 %and 52. 78 % respectively, and the specificity were 100 %,98. 11 %and 98. 86 % respectively. The positive predicting value of cell culture, PCR, and Colloidal Gold was 100 %,86. 84 % and 86. 36 % respectively, while the negative predicting value was 96. 35 %,98. 85 %and 93. 88 % respectively.For experimental group , infertility group,ectopic pregnancy group ,chronic pelvic pain and recurrent PID group and control group, the chlamydia positivity was 28.30%(60/212),26.19%(22/84),28.33%(17/60), 30.88%(21/68) and 7.57%(5/66) respectively.A significant difference was observed between the control group and experimental group,including infertility group,ectopic pregnancy group ,chronic pelvic pain and recurrent PID group.Conclusion:1. Colloidal Gold is one kind of simple, quick and highly specificity CT examination method, the recommendation uses in diagnosing the feminine genital tract CT infection.2. The chlamydia positivity is 28.30% in sequela of PID ,the infection with CT is one of the causes of sequela of PID.
