Objective: The study is to survey retrospectively the HIV infection old in 50 years age or older, including of general population index and dangerous behavior etc, in order to clarify the characteristics, reasons and transmission routes in the HIV infection old groups and provide a scientific basis for the development of appropriate control measures, to reduce HIV infection rate among the old.Methods: The integrated control data was from Centre for Disease Control and Prevention in Hunan, surveyed 80 persons who was positive in HIV antibody test and 50 years old or older from four cities (county, district) of western region, and randomly selected 83 common old people in the local as a control group. The investigation includes sexual behavior, psychological factors, economic status, health, infection causes and factors such as social activities of the old people. The investigators were trained and survey the old under the principle of informed consent by face to face, one to one. Enter EPIdata3.02 findings to establish a database, General population index and dangerous behavior etc was analyzed by SPSS 13.0.Results:1. Basic conditionThe average age of 80 of elderly HIV patients was 65.2 years old, the minimum was 51 years old and the maximum was 82 years old. The number of male was 46 and female was 34. There was 76.3% primary school education, 70% farmers, 52.5% resting at home, 53.8% separated from spouses of the surveyed old people. In control group, there were 52 males and 31 females, average age was 64.8 years old, the rate of primary school education or less was 66.3% , farmers was 68.7%, resting at home was 50.6%, and separated from spouses was 13.3%. There was not statistically significant different between the HIV infection group and control group in age, gender, ethnic, occupational characteristics. There was comparability between the two groups.2. Transmission routes of infection groupThe 80 elderly HIV / AIDS patients had no history of drug addiction. There were 38 of 46 male patients(82.6%) who admitted having sexual relationship with the commercial sex partners, 2 patients had blood transfusion and 1 patient had the history of selling blood. The leading transmission route of elderly men was from commercial sexual partners. There were 27 of 34 female patients (79.4%) living with HIV-positive spouse. The female patients were denied extramarital sexual behavior, no history of selling blood, and 5 patients had blood transfusions. The main transmission route of elderly women was from spouses.3. Risk factors of infectionThe rate of the sexual behavior with commercial sex partners was 82.6% in elderly male HIV group. The main place was relatively concentrated, cheap, such as rental housing. In control group, the rate of dangerous sexual behavior with commercial sex partners was 5.8%. There was significantly between the HIV infection group and the control group (P = 0.000).There were three major risk factors of elderly men. Firstly, they had little or a little knowledge of prevention of AIDS, 82.6% of men in the infection group didn't use condom in sexual behavior, 94.7% of patients never used condom when they make sex with commercial sexual partners. There was significant statistically different between the HIV infection group and control group (P <0.01). Secondly, They didn't satisfy the sex life within marriage or long-term separation. In infection group, The rate of less than twice sex in one month is 45.6%. There was significant different between the male group and the control group within marriage in sexual satisfaction (P <0.01). Lastly, they lacked the healthy and beneficial cultural and sports activities. The 42 elderly male patients'common activities is monotonous in infection group(accounting for 91.3%).There was significant difference between HIV infection group and the control group (P <0.01). The elderly female HIV patients infected was mainly from spouses. its risk factors were mainly that the spouse failed to detected after HIV infection, usually didn't use condoms in sexual intercourse. In the infection group, 69 patients didn't use condom within marriage (86.6%). In addition to, they were affected by the traditional sexual attitudes and cultural.Conclusion1. The mainly transmission elderly men with HIV infection was from commercial sexual partners. Local elderly lack of HIV / AIDS knowledge, Most of the elderly sex demand imbalance, and they lack the health benefits of cultural and sports activities, there are a lot of free time, to encourage older men occur risky sexual behavior with commercial sex partners is the local elderly men infected with HIV/ AIDS a major factor. The main way of elderly women with HIV infection was from their spouses. The local men infected during sex without taking any safety measures, followed by the male sexual behavior will mainly be an important factor leading to infection.2. To deeply promote the knowledge of HIV of elderly person prevention and behavior intervention and organize the elderly persons to carry out healthy and beneficial cultural and sports activities. The fundamental measure to prevent the HIV infection of elderly person is banning commercial establishments in society.
