ObjectiveTo investigate the gender, age and distribution changes of different types of hospitalized glaucoma patients, especially for primary glaucoma patients.MethodsThe hospitalized glaucoma cases (1212 cases) in Tianjin medical university eye center from January 1989 to November 2003 were retro-spectively enrolled in this study and were classified by the diagnosis, sex and age. The proportion of different types of glaucoma was calculated and the changes of the proportion were analyzed. The data of different period (1989.1 to 1997.12,1998.1.1 to 2002.12, and 2003.1 to 2007.12) were also analyzed.ResultsDuring the nineteen years, there were 876 primary angle-closure glaucoma patients (72.3%),99 primary open-angle glaucoma patients (8.2%),234 secondary glaucoma patients (19.3%)and 3 congenital glaucoma patients (0.2%). For primary angle-closure glaucoma patients, acute primary angle-closure glaucoma accouted for 42.1%(369/876), and 57.9%(507/876) for chronic primary angle-closure glaucoma.' For the three periods:1989.1-1997.12,1998.1-2002.12 and 2003.1-2007.12, the percentage of acute angle-closure glaucoma in primary glaucoma patients were 0.5%, 8.6% and 28.7%, respectively; the percentage of chronic angle-closure glaucoma were 1.4%,14.7% and 35.9%, respectively; the percentage of open-angle glaucoma were 0%,2.4% and 7.8%, respectively. The data showed an upward trend of primary glaucoma hospitalization treatment. For different age groups, the women over 40 years old with acute angle-closure glaucoma showed an upward trend, and the men over 40 years old with open-angle glaucoma also showed an upward trend. Fot the two periods:1998.1-2002.12 and 2003.1-2007.12, the average age of acute angle-closure glaucoma patients were 65.67 and 67.07 years, respectively, the average age of chronic angle-closure glaucoma patients was 65.42 and 68.38 years, and 54.78 and 61.34 years for open-angle glaucoma patients. The ratio of male to female was 1:1.47 for angle-closure glaucoma patients; 1:2.69 for acute angle-closure glaucoma patients; 1:1 for chronic angle-closure glaucoma patients and 1:0.68 for open-angle glaucoma patients.ConclusionsPrimary angle-closure glaucoma is still the main type of hospitalized patients with glaucoma. However, the percentage of primary open-angle glaucoma seems to be rising during those years. It is essential to pay more attention to primary open-angle glaucoma The increasing age of angle-closure glaucoma may be associated with the population aging process, as well as the socio-economic factors. The relativley young age of open-angle glaucoma patients may be related to the genetic characteristics. We should pay more attention to women over 40 years during and sure glaucoma screening, especially for chronic angle-closure glaucoma screening.
