Objective:To make the hydronephrosis model of newborn Wistar rats in 48 hours after birth by psoas oppression ureter. During the making ureteral obstruction and reliefing ureteral obstruction, to detect respectively the transcription level and protein level changes of transforming growth factor-β1 (TGF-β1), monocyte chemotactic factor-1(MCP-1), endothelin-1 (ET-1), insulin-like growth factor-1 (IGF-1) in hydronephrotic kidneys(HnK) and its-concentration in the renal pelvis urine.To explore true indicator of evaluateing damage degree from cytokine in HnK and its' recovery degree.And to provide experimental evidence in favour of the choiceing treatment method in pediatric hydronephrosis and predicting operation effect.Methods:1,To make animal model:to make the animal model of congenital hydronephrosis (to make the partial ureter obstruction (PUO) by psoas oppression ureter and relief ureteral obstruction).The 72 newborn Wistar rats in 48 hours after birth were randomly divided into 6 groups and each group was 12 Wistar rats. To selected randomly 1 group as sham group and the other 5 groups as experimental groups. Wistar rats were sacrificed 3 weeks after PUO in group 2. And group 3 was 2 weeks after reliefing ureteral obstruction(RUO), group 4 was 6 weeks after RUO, group 5 was 8 weeks after RUO, group 6 was 12 weeks after RUO.2,To examine experimental maker:(1) To examine histology changes in HnK by HE staining, and make pathological grade; (2) To examine the transcription level and protein level changes of TGF-β1,MCP-1,ET-1,IGF-1 in HnK during the ureteral obstruction and reliefing ureteral obstruction. (3)To detect the concentration changes of TGF-β1,MCP-1, ET-1,IGF-1 in the renal pelvis urine during the ureteral obstruction and reliefing ureteral obstruction. Results:1,To make successfully hydronephrosis model in Newborn rats.All cases were graded by HE staining according to Elder standard. GradeⅠin sham group. And gradeⅠ~Ⅴin experimental groups(9 cases in death and 51cases in survive), gradeⅠin 7 cases, gradeⅡin 13 cases, gradeⅢin 20 cases, gradeⅣin 11 cases, gradeⅤin 0 cases. Histological changes in HnK may gradually improved with the extension of the reliefing ureteral obstruction.2,The expression changes of TGF-β1,MCP-1,ET-1 mRNA in HnK:(1) The mRNA expression of TGF-β1,MCP-1,ET-1 were gradually increased with the pathological grade increasing.There was a significant difference for TGF-β1 of hydronephrotic kidneys in the different pathologic grades (P<0.05),whereas the was partlly difference for MCP-1,ET-1 in pathologic grades and so was IGF-1 though the IGF-1 expression was decreased with pathological grade increasing. The protein expression of TGF-β1,MCP-1,ET-1 were gradually increased with pathological grade increasing except for IGF-1.(2) After reliefing ureteral obstruction,the expression of the transcription level and protein level in IGF-1 was gradually increased with RUO extension,whereas the expression of TGF-β1,MCP-1,ET-1 were gradually decreased and TGF-β1 was normal level in 12 weeks after RUO.3,The concentration changes of TGF-β1,MCP-1,ET-1 in renal pelvis urine:(1) The concentration of TGF-β1,MCP-1,ET-1 in the renal pelvis urine were gradually increased with pathological grade increasing. There was a significant difference for urinary TGF-Pi in the different pathologic grades (P<0.05), whereas the was partlly difference for urinary MCP-1,ET-1 in pathologic grades and so was IGF-1 though the IGF-1 level was decreased with pathological grade increasing.(2) After reliefing ureteral obstruction, the concentration of IGF-1 in the renal pelvis urine was gradually increased with RUO extension,whereas the concentration of TGF-β1, MCP-1,ET-1 were gradually decreased and TGF-β1 was normal level in 12 weeks after RUO.Conclusion:1,To make successfully hydronephrosis model in Newborn rats by by psoas oppression ureter and relief ureteral obstruction.And it may be the ideal animal model for pediatric surgical doctor to study congenital hydronephrosis.2,The transcription level and protein level expression of TGF-β1 in HnK in Newborn Wistar rats was a significant difference at the different pathologic grades (P<0.05), and TGF-β1 was a normal level 12 weeks after RUO.And it may be used for evaluating the damage degree in HnK and recovery status.3. The concentration of TGF-β1 in the renal pelvis urine was a significant difference at the different pathologic grades (P<0.05), and TGF-Pi concentration was a normal level 12 weeks after RUO.And it may be an ideal indicator for evaluating the damage degree in HnK and recovery status.
