ObjectiveTo explore the percentage of CD4~+CD25~+Treg in peripheral blood from patients with chronic hepatitis B and detect the levels of IL-2,IL-4,IL-10,IFN-γand TGF-βin serum and investigate the roles of CD4~+CD25~+Treg and cytokines in blood on the chronicity of hepatitis B.Methods1 Selecting 24 cases of chronic hepatitis B (CHB) as the CHB group, 8 cases of acute hepatitis B (AHB) as the AHB group who are from the Department Of Infectious Diseases of Peking University Shenzhen Hospital and 12 healthy persons as the normal control group who are from Health Examination of Peking University Shenzhen Hospital. Detecting the percentage of CD4~+CD25~+Treg in peripheral blood of the three groups with flow cytomerty;2 Detecting the levels of IL-2, IL-4, IL-10, IFN-γand TGF-βin the groups of chronic hepatitis B and the normal control with double antibody sandwich method of ELISA;3 Measuring the serum of HBV-DNA in the groups of CHB and AHB with real-time RT-PCR;4 Measuring the serum of ALT in all of the groups with AU5400 biochemistry analyzer.Results1 The percentage of CD4~+CD25~+Treg in the three groups are respectively as followed: 14.93±0.60% in the group of CHB, 8.64±0.75% in the group of AHB, 9.06±0.35% in the group of normal control. The percentage of CD4~+CD25~+Treg in the group of CHB is obviously higher than that of the group of AHB and that of normal control (both P<0.01). However, there is little discrepancy between the groups of AHB and normal control (P>0.05). There is no correlation between CD4~+CD25~+Treg and HBV-DNA both in the groups of CHB and AHB (r=0.232, P>0.05; r=0.34, P>0.05). Also, there is no correlation between CD4~+CD25~+Treg and ALT both in the groups of CHB and AHB(r=0.196, P>0.05; r=-0.368, P>0.05);2 The levels of IL-2, IL-4, IL-10, IFN-γand TGF-βin the group of CHB are 31.27±3.16pg/ml, 52.12±2.65pg/ml, 165.78±11.33 pg/ml, 76.25±1.70 pg/ml and 10.96±0.83 pg/ml respectively, while in the group of normal control are 14.68±1.77 pg/ml, 25.06±2.66 pg/ml, 84.49±3.21 pg/ml, 43.00±2.92 pg/ml and 5.73±0.64 pg/ml respectively. The level of each cytokine above the fine ones in the group of CHB is higher than that of the group of normal control (P<0.01). There is a positive correlation between ALT and IL-2,IFN-γrespectively in the group of CHB(r=0.442,P<0.05;r=0.469,P<0.05).Conclusion1 There exists immunologic derangement in patients with chronic hepatitis B;2 CD4~+CD25~+Treg in peripheral blood and Th1/Th2 imbalance play an active role in the chronicity of hepatitis B.
