Clinical Analysis Of Chronic Renal Insuffience In Children

Objective:1,to learn the cause constitution,variation and clinical characteristics of chronic renal insufficiency in children , provide guidance for diagnosing and giving intervention at the early stage of insidious renal disease , and improve the prognosis。2,To explore the role of urinary screening and ultrasound screening programmes in screening urologic diseases in asymptomatic children,the results of urinary analysis and renal ultrasonography of asymptomatic children in urban and rural area of ChongQing has been analyzed。Approach:1,we collected 60 cases of patients with CRI in our hospital from January 2004 to december 2009,analysised retrospectively their etiological factors,clinical characteristics,treatment and the prognosis2,Statistical analysis has been carried on to the results of urinary analysis and renal ultrasonography in our hospital in the asymptomatic children involved in the event of problemed powdered milk from September to November in 2008. Result:1,there are 39 cases of aquired renal glomerular diseases ,which consisit 65%of the total cases ,and the mean age of onset is 11.07 years old ,mean course of disease before diagnosis is 1.48years .2,20cases of congenital and inherited renal diseases consist 33.33%of the total cases ,the mean age of onset is 8.41years old ,the mean course of diseases before diagnosis are 4.25years .3,the main clinical characteristics are renal anemia,hypertention,electrolyte disorders,metabolic acidosis,retardation of growth and development,infection .4,all the ailing children suffer from anemia ,and the hemoglobin has a converse relationship with serum creatinine (r=-0.526,P<0.001).5,CRI usually begins with few symptoms ,55%of the patients are found renal failure even the end-stage of renal failure when diagnosed .6,572 cases of abnormal urinary sample has been detected.In which ,569 cases were detected by urine dipstick:including 218 cases of leukocyturia ,40 cases of albuminuria ,354 cases of occult blood positive;239 cases were detected by urinary microscope examination.7,409 cases of abnormal urinary system were detected by the ultrasonography of urinary system,containing 97 cases of congenital abnormality of renal pattern and structure.Conclusion: 1,compare with the etiological factor constitution of children in our hospital 6 years before ,ranl glomerular diseases decreased ,while congenital and inherited renal diseases increased ,but aquired renal glomerlular diseases are still the main causation .2,in review of the insidious onset and diversified clinical characteristics , diagnosis and treatment at the early stage must be emphasized to prevent or delay the process to the end-stage of renal failure,and improve the life quality。3,Routine urinary analysis and renal ultrasonography should be adopted in healthy children to detect various asymptomatic urinary disorders at an early stage and reduce the incidence and development of chronic renal disease.
