| Objective:By analyzing the related factors about apnea of prematurity, to explore the high risk factors, to monitor and take precautions against premature children with these high risk factors in clinical.Methods:Retrospective analysis was done in the apnea and non-apnea in premature children of all 94 cases in each group of complete clinical data in Neonatology Division of children's hospital of Chongqing medical university from January 2008 to December 2009, respectively as the case group and the control group. Univariate analysis a variety of related factors, then logistic regression analysis factors with univariate analysis was statistically significant.Results:1. There are 29 cases of primary apnea and 65 cases of secondary apnea in 94 cases of AOP. Primary apnea occurred in the first 2-7 days after birth accounted for 65.5%, 34.5% in the first day; secondary apnea can occur at any age segment.2. The smaller the gestational age, the higher the incidence of primary apnea, as the gestational age, secondary and primary apnea compared the incidence of significant difference(P<0.01). In the secondary factors, pulmonary diseases and central nervous system disease rate was 64.6% and 60.0%.3.Univariate statistical analysis of 20 related factors showed that: gestational age, birth weight, pneumonia, hyaline membrane disease, brain damage in premature children, neonatal scleredema, neonatal severe anemia, acidosis, and hyponatremia had statistical significant difference between case group and control group(P<0.05).4.Multivariate analysis showed that less than 34 weeks gestational age (OR=1.222,95%CI 1.048~1.424), brain damage in premature children (OR=5.328,95%CI2.669~10.635),acidosis(OR=2.623,95%CI 1.294~5.317), hyponatremia (OR=12.280,95%CI 1.208~124.819), P values were P<0.05,P<0.01,P<0.01,P<0.05.Conclusions:1. When apnea occurred was related to its type. Secondary apnea can occur in postnatal any day; primary occurred more frequently in postnatal 2 ~ 7d, rarely occurred in the 7days after birth.2. Types of apnea related to gestational age. The smaller the gestational age,the higher the incidence of primary apnea. The most common causes of secondary apnea were pulmonary diseases and central nervous system diseases.3. Gestational age is less than the 34th week,premature child brain injury, acidosis and hyponatremia occurring as the AOP related risk factors.4. Premature with the above risk factors should be on guard apnea. Focused on monitoring and timely treatment of AOP to reduce mortality and avoid complications. |
