Objective To systematic review the accuarcy of Cytokeratin 20 (CK20) in peripheral blood in detecting colorectal neoplasm.Methods Cochrane systematic evalution was used to search through MEDLINE, EMBASE, Cochrane Library, CMCC, CNKI for studies regarding CK20 in peripheral blood in the diagnosis of Colorectal neoplasm. Data were extracted and analyzed using software MetaDiSc1.4.Results In total, 12 relevant studies were searched. Heterogeneity, expect for threshod effect, was found within these studies of CK20. By methodological qualities and Q test, 12 papers were selected into this study. The overall diagnostic sensitivity, specificity, and area under curve (AUC) from receiver operating characteristic (ROC) curve were adopted to evaluate CK20 as the diagnostic standard for colorecyal carcinoma.Coclusions CK20 could be uesd as a adjuvant diagnostic marker of colorectal cancer. CK20 tested by RT-PCR could distinguish healthy volunteers from colorectal carcinoma,its AUC was 0.8637. CK20 tested by FQ-PCR and modifed RT-PCR, could distinguish benign bowel disease from colorectal carcinoma. And its AUC was 0.9565, which was much better than RT-PCR. The positive ratio of CK20 was associated with tumor stage.
