Objective: Complication after total laryngectomy and hypopharynx malignant tumor were frequent. To detection the risk factors of pharyngeal fistula in patients with total larungectomy. Methods: To retrospectively analyze that clinical data from January 1999 to November 2009, 96 patients with pharyngeal fistula were treated by total larynhectomy. To analyze age, sex, tumor part, clinical stage of tumor, incision of trachea, patho-staging, pre-operative radiotherapy, cell differentiation degree, pre-operative nutrition states and appearance of post-operative infection with Logistic model. Result: The occurrence rate of post-laryngectomy pharyngo-cutaneous fistula was 22.9 %. By analyze with Logistic model and Single factor analysis showed that parameters related to the occurrence rate of post-laryngectomy pharyngocutaneous fistula included clinical stage of tumor, appearance of post-operative infection, undergoing of pre-operative radiotherapy, and pre-operative nutrition showed that pharyngeal fistula's the occurrence rate of after total laryngectomy pharyngeal fistula included clinical stage of tumor, undergoing of pre-operative radiotherapy and pre-operative nutrition states (p<0.05). Conclusion: patients with tumor and who underwent pre-operative radiotherapy may contribute toward increasing the possibility, favourable preoperative nutrition may contribute toward reducing the incidence of pharyngeal fistulas; Postoperative laryngeal fistula is related to many factors, avoiding the above mentioned factors can reduce the rate of fistula and improve the life quality. |
