Study On Application Of Amlodipine Combining With Telmisartan Or Co-amiloride In Patients With Mild-moderate Hypertension

Objective: To compare control rate of blood pressure,renal function,metabolism of glucose and lipid and blood uric acid in subjects with early and early-middle hypertension adminstrated amlodipine combined with telmisartan and co-amiloride, respcetively.Methods: 154 subjects with hypertension were randomly divided into two groups :subjects in group A were adminstrated amlodipine and co-Amiloride; subjects in group T were adminstrated amlodipine and telmisartan. There is no significance of difference in blood pressure in baseline. All subjects had been treated for 12 months. Mean blood pressure was recorded at 1 week,2 weeks,1 month,2 month,3 month,6 month,9 month,and 12 month respectively,and then control rate of blood pressure,renal function, electrolytes, metabolism of glucose and lipid and blood uric acid were compared simultaneously.Results: 1. 75 subjects were distributed randomly in group A and 79 subjects in group T. Basic characteristics of groups were similar and random distribution was optimal. Two data of groups had comparability. 2. Control rates of blood pressure in two groups were 44.7% and 46.5% respectively after all subjects were treated for two weeks. Control rates were 8.9%,59.5%,74.1%,74.6%,79.1%,和80.0%,82.5% and 83.6% respectively at 1,2,3,and 6 month;Control rate in group T was higher than that in group A,but there was no significance of difference in two groups. Control rate in group T was higher than that in group A and there was significance of difference in two groups. 3. eGFR had no significance of difference in two groups. 4. Blood cholesterol,HDL,LDL and TG in two groups had no significance of difference pro and post treatment,at 12 month(P>0.05).5. uric acid in groups A increased 10.6±70.7μmol/L, instead, uric acid in group T decreased 10.25±50.9μmol/L between at 12 month and random phase(P=0.034).Conculsion:1.There are obviously therapeutic effects in control of blood pressure in two regime.Blood pressure can be controled in 95% percent of patients with mild-moderate hypertension.There is no significance of difference between two therapeutic regimen in two groups. Amlodipine combined with telmisartan is considered to as a optimal regimen for these early-middle hypertension. 2. there is no significance of difference in renal function and metabolism of glucose and lipid in two groups. Amlodipine combined with telmisartan has no bad influcence in blood uric acid but amlodipine combined with co-amiloride leads to increase of blood uric acid. Ventricular reconstruction, arterial sclerosis and cerebrovascular accident should evaluate further in these patients.
