Objective:The Serial section, Immunohistochemistry and FQ-PCR were applicated to detect the incidence of cervical lymph node micrometastasis in patients with cNo supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma, in order to accurate pathological level and guide the treatment and judge prognosis properly. The specific and highly sensitivity markers of tumor micrometastasis was studied. The correlation between different syndromes of patients with cNo supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma and cervical lymph node micrometastasis was discussed.Methods:The lymph nodes taken from neck dissection specimens in 25 patients with cNo supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma were prospectively evaluated by Serial section, Immunohistochemistry and FQ-PCR assay. CK14 expression in lymph nodes from various districts was compared among FQ-PCR, serial section and immunohistochemistry. Then a cross-check analysis was executed to evaluate the sensibility and specificity of each method. The incidence of lymph node micrometastasis in different neck partition was studied. The correlation between different syndromes of patients with cNo supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma and cervical lymph node micrometastasis was analyzed.Results:1) The rate of cervical lymph node micrometastasis by FQ-PCR was 56%(14/25), higher than that by serial section and immunohistochemistry. There are statistically significant (P<0.05).2) The rate of lymph node micrometastasis in patients of T1, T2 stage was 36.4%(4/11). The rate of lymph node micrometastasis in patients of T3, T4 was 71.5%(10/14). cN0 supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma of T stage and neck lymph node micrometastasis were relevanted according to analysis. (P<0.05)3) Syndromes was associated with lymph node micrometastasis (P<0.05).Conclusion:1) To detect micrometastasis in lymph nodes by FQ-PCR was a feasible method.2) The neck lymph node micrometastasis of cN0 supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma tend to occur inâ…¡,â…¢area.3) The T stage of cNo supraglottic laryngeal squamous cell carcinoma and hypopharyngeal squamous cell carcinoma was closely related to neck lymph node micrometastasis4) Syndromes and neck lymph node micrometastasis were relevant.
