Objective: Mifepristone as a strong antiprogestin , has been widely used to terminate early pregnancy in clinical. At present, its mechanism of anti-early pregnancy has developed to the genetic level, but the mechanisms are not yet entirely clear. In pregnancy, placental microvascular formation and its network of blood vessels to build for the fetal development has a crucial role. Mifepristone is widely used in clinical termination of the current 40-49days of pregnancy. On the mechanism of anti-early pregnancy with mifepristone research and reports, its object of study are mostly of early pregnancy 40-49 days. It remains to be further studied whether the mechanisms of anti-early pregnancy are the same between the crowd for early pregnancy≤39 daysand early pregnancy 40 -- 49 days.This study, by immunohistochemistry and fluorescence quantitative PCR, observe the effects of Mifepristone on the expression of VEGF,VEGFR-2(KDR) in human decidua and chorion villi for different early human pregnancy. For clinical use earlier and provide theoretical basis for the termination of early pregnancy.Method:1 All the specimens were collected from the women who desire to terminate the early pregnancy in the department of gynecology and obstetrics of Hebei people hospital from October 2008 to April 2009. The forty women were divided into two groups : the control group, 20 cases for 40-49 days of early pregnancy menopause women; the study group, 20 cases of menopausal≤39 days of early pregnant women.2 Detect the expression of VEGR and KDR in decidua and chorion tissue by Immunohistochemical methods and fluorescence quantitative PCR. 3 Analyze the data with SAS.Results:1 Light microscope showed (HE stain):The decidua tissue of control group contain decidual cell and granular cell. The structure is complete and the rank is compact. The decidual cell turn to degenerate and necrosis, interstitial edema, blood capillary expand and engorge, inflammatory cell infiltrate and the cell bleed. The morphological change of study group is basically alike to the control group.The structure of chorion trophoblastic cell of control group is clear, which the outer layer is syneytiotrophoblast and the inner layer is langhans cell. The chorion trophoblastic cell showed: the cell degenerate, necrosis, even amotic, the inflammatory cell infiltrate, the chorion interstitial edema, the blood siltation and the fibrin deposition. The change of study group is basically alike to the control group.2 the expression of VEGF in decidual and villous tissuesIn decidua tissue of control group, the expression of VEGF locate in the majority of decidual cell, and the positive granules were located within the nucleus and a small amount of expression in cytoplasm. VEGF expression in the study group compared with the control group, is no statistical significance (P>0.05).In chorion tissue of control group, the expression of VEGF is in the nucleus and cytoplasm of the chorion cell. The expression of VEGF expression in the study group compared with the control group, is no statistical significance (P>0.05).3 The expression of KDR in decidua and chorion tissueIn decidua tissue of control group, the expression of KDR is in the nucleus and cytoplasm of the majority of decidual cell and the minority of glandular epithelium cell. Comparing the KDR expression of study group with control group is no statistical significance (P>0.05).The expression of KDR is positive in the mainly in the nucleus and cytoplasm of the chorion cell of control group. The KDR expression of study group is similar to the expression of control group and there is no statistical significance between them(P>0.05).4 The test rusults of VEGF in the decidua, chorionic tissue by fluorescence quantitative PCRThe rusults of VEGF in the decidua by fluorescence quantitative PCR determination, and the two sample t-test confirmed: comparing decidual VEGF expression in the study group (0.8901±0.1817) with the expression in the control group (0.9699±0.3134) is no statistical significance (P>0.05).The rusults of VEGF in the chorionic by fluorescence quantitative PCR determination, and the two sample t-test confirmed: comparing decidual VEGF expression in the study group (1.0035±0.1907) with the expression in the control group (1.2105±0.3386) is no statistical significance (P>0.05).5 The test rusults of KDR in the decidua, chorionic tissue by fluorescence quantitative PCRThe rusults of KDR in the decidua by fluorescence quantitative PCR determination, and the two sample t-test confirmed: comparing decidual KDR expression in the study group (0.4425±0.1989) with the expression in the control group (0.5273±0.1155) is no statistical significance (P>0.05).The rusults of KDR in the chorionic by fluorescence quantitative PCR determination, and the two sample t-test confirmed: comparing decidual KDR expression in the study group (0.5414±0.1499) with the expression in the control group (0.6232±0.2179) is no statistical significance (P>0.05).Conclusions:1 VEGF, KDR were expressed in decidua and chorion tissue of early pregnancy≤39 days and early pregnancy 40-49 days. the expression of VEGF locate in the majority of decidual cell nucleus, and the cytoplasm is also a small amount of expression. Chorion cells were expressed within the nucleus and cytoplasm, too. The expression of KDR is in the nucleus and cytoplasm of the majority of decidual cells and the minority of glandular epithelium cells and nucleus and cytoplasm of chorion cells.2 Mifepristone can inhibit the expression of VEGF, KDR in decidual and chorion tissues, and undermine the normal role of VEGF, KDR in early pregnancy, so pregnancy can't maintain and secondary abortion. However, the change of VEGF, KDR is no statistical significance between the study group (early pregnancy≤39 days) and the contron group (early pregnancy 40-49 days). Theoretically, mifepristone has the same effect to the deciduas and chorion tissue between early human pregnancy≤39 days and 40-49 days.
