Objective:To investigate the effects of puerarin preconditioning on expression of nuclear transcription factor-KB (NF-κB), pro-inflammatory cytokines IL-6 and IL-8 in neutrophil of patients during valve replacement under cardiopulmonary bypass(CPB) for exploring the potential cellular and,molecular mechanisms of its anti-inflammatory effects and theoretical basis for clinical application.Method:40 patients with rheumatoid heart disease (RHD) and undergoing valve replacement under CPB were enrolled and randomly divided into experimental group (puerarin group,P group)(n=20) and control group(NS group, C group)(n=20).Puerarin 4mg/kg (in NS 10ml) in 5%GS 250ml was infused intravenously in P group, and 10 ml NS was infused the same way in C group before CPB.4ml whole blood (WB) sample was then collected at four time points:before CPB(T0),and 30min (T1),1h (T2),2h (T3)after aorta declamping. The expression rates (%) of NF-κB,IL-6 and IL-8 in PMN were assayed with flow cytometry.Results:1.The expression of NF-κB and IL-6,IL-8 significantly increased at T2,T3 compared with To (p<0.05).There were no statistical differences on the expression of NF-κB and IL-6,IL-8 in P group at T1, T2 and T3 (p>0.05).The expression of NF-κB increased gradually from T1 to T3 in C group, and remarkablely at T3 compared with T1(p<0.05). The expression of IL-6 and IL-8 also increased from T1 to T3 in C group, and remarkablely at T2,T3 compared with T1(p<0.05).2.There were no statistical difference found on expression of NF-κB and IL-6,IL-8 between the two groups at the time of T0(p>0.05).The expression of NF-κB and IL-6,IL-8 were significantly lower at T3 in P group than in C group (P<0.05).The total positive expression of NF-κB and IL-6,IL-8 were significantly lower in p group than in c group (p<0.01).Conclusion:1.The production of NF-κB,IL-6 and IL-8 in neutrophil of patients with RHD during valve replacement under CPB increases after myocardial ischemia and reperfusion.2.Puerarin preconditioning may attenuate over-production of NF-κB and pro-inflammatory cytokines such as IL-6 and IL-8.
