Comparative Study On Clinical Effect Of Cortical Screw And Lag Screw In Treatment Of Acute Separation Of Tibiofibular Syndesmosis

ObjectiveThrough the study on acute separation of tibiofibular syndesmosis (including accompanied ankle fractures and not accompanied) in Changzhou hospital of Traditional Chinese Medicine by the last two years, we have observed and analyzed the difference between cortical screw and lag screw on clinical effect, functional outcome and complication, then we can provide experience for the treatment of acute tibiofibular syndesmosis separation.MethodThe design is a clinical study. We collected the cases from Changzhou hospital of traditional Chinese medicine. We screened and brought into some acute tibiofibular syndesmosis separation cases which fitted our standard to fill in tibiofibular syndesmosis separation survey form (attached table 1),then we followed up the cases and filled in tibiofibular syndesmosis separation revisit form (attached table 2).Afterwards we input collected information to computer.Finally, all the data was analyzed by the software named spss 17.0.Result1.According to integrates, the elimination and the diagnosis standard, thirty-one cases were brought into our research (including eighteen cases were treated with cortical screw and thirteen cases were treated with lag screw). There were no significant difference between the two groups with respect to demography and clinical baseline characteristics.2.There were obvious significant difference between the preoperative X-ray and postoperative X-ray of the first day which were respected to tibiofibular space. All the cases were examined postoperative X-ray of 2weeks and 12 weeks, and there were no significant difference among three times.3.There were no significant difference between the two groups with respect to functional outcome.4.There was a screw breaked which fixated with lag screw, and nothing happened in the group of cortical screw.In the lag screw group, there were more cases suffered residual ankle pain than the cortical screw group.Conclusion1.The curative effects of tibiofibular syndesmosis separation were very well with both cortical screw and lag screw, and they kept tibiofibular syndesmosis in the right location steady.2.There were no significant difference between the two groups with respect to functional outcome. But in the lag screw group, there were more cases suffered residual ankle pain than the cortical screw group.Therefore, the cortical screw was superior to lag screw.This research provided foundation for the treatment and prognosis of acute tibiofibular syndesmosis separation.
