To evaluate urinary albumin excretion rate(UAER) in women with polycystic ovarian syndrome (PCOS) in relation to their clinical, endocrine, and metabolic indexes.Based on ESHRE/ASRM criteria, we collected 100 patients with polycystic ovary syndrome and 30 healthy women as control. Using case-control study on them with clinical features, serum sex hormone levels, urinary albumin excretion rate, metabolic indicators and the correlations between them.1.The clinical featuers are summerized as follows:oligomenorrhea take up 63%,secondary-amenorrhea take up 12%, menstrual disorder take up 13%, hyerandrogenism take up 59%,69% PCOS with LH/FSH≥2,37%PCOS with LH/FSH≥3,44% patients have acne,19% patients are hirsutism,78% show polycystic ovary one or both side toword B-ultrasound,25% with the ovarian volume≥10ml,43% patients are obesity (BMI≥25kg/m2),38% patients are central obesity (WHR≥0.85),49% have insulin resistance,17% have acanthosis nigricans,36% have metabolic syndrome.2.Among them, typeⅠPCOS (PCO/HA/IM)take up 40%, typeⅡPCOS (PCO/HA) take up 9%, typeⅢPCOS (PCO/IM) take up 36%, typeⅣPCOS (HA/IM) take up 15%.3. Adolesent patients (age≤19 years) take up 12%, reproductive age patients take up 88%,41 cases of patients with infertility or secondary infertility, occupied 46.59% of the reproductive age patients.4. The menstrual cycle,BMI, WHR, hirsutism score, ovarian volume, antral follicle number, T, A2, DHEA-S, FTI, LH, LH/FSH, FPG,FINS,TG,TC,HOMA-IR of PCOS are higher than healthy control group.SHBG, HDL are lower than the control group, the differences are statistically significant (P<0.05).5.59 PCOS patients have UAER testing, the incidence of microalbuminuria in this group is 8.47%-13.6%, in the control group, the incidence of microalbuminuria is 6.67%. There is no statistical difference between the two groups (P>0.05)6. The urinary albumin concentration and urinary albumin/creatinine rate(ACR) of the PCOS group are higher than the control group, the difference is statistically significant (P<0.05).7. Pearson correlation analysis indicated that, ACR positively correlate with diastolic blood pressure, BMI,WHR,TG,HOMA-IR, and negatively correlate with HDL (P<0.05). Multiple linear stepwise regression analysis showed that,in the P< 0.05 level, diastolic blood pressure and HOMA-IR into the equation, and as the independent risk factors of UAER in patients with PCOS.1. PCOS patients concentrated in the age of 20 to 35 years old.2. ESHRE/ASRM criteria is mainly suitable for the PCOS patients in our region. PCOS patients with LH/FSH≥2 take up a great proportion,so LH/FSH≥2 can be a secondary diagnosis for the PCOS in this area.3. TypeⅠand typeⅢare more, which is different from other foreign and domestic research.4. The incidence of microalbuminuria in PCOS is 8.47%-13.6%.5. The urinary albumin concentration and urinary albumin/creatinine ratio (ACR) in PCOS are higer than the control group, suggesting PCOS have higher UAER trend.6. The UAER in PCOS has relation with some clinical features, among them HOMR-IR and diastolic blood pressure as the independent risk factor of UAER...
