Objective:To investigate the benign and malignant lesions differentiation by relativity analysis between solitary pulmonary nodule (SPN) and bronchus with multi-slice spiral CT (MSCT).Materials and Methods:Seventy-three patients (men,46cases and women,27 cases; age range,11-79 years; mean±SD,47.1±15.9 years) were selected according to discovery of SPN and bronchus relativity on MSCT images, pathological and clinical following results. The nodule was defined as less than 30mm diameter. Among 73 patients,109 bronchi were identified as SPN-related bronchus, with 46 bronchi in benign SPN and 63 bronchi in malignant SPN.MSCT application:1. Eight millimeter slice thickness was used in spiral scanning. Raw data was reconstructed by multi-planar reformation (MPR) and curved planar reformation (CPR) with bone algorithms.2. SPN-related bronchus was classified to five types according to SPN and relativity bronchi.3. The contrast analysis was conducted between SPN-related bronchus and pathological and clinical following results.Results:In five types of SPN-bronchus relationship, typeⅠ,ⅡandⅣare relatively more common. TypeⅠ(38.1%),Ⅳ(30.2%) andⅡ(22.2%) are commonly found in malignant nodule and typeⅠ(37%),Ⅱ(30.4%) andⅤ(19.6%) in benign nodule. No significant differentiation was found in typeⅠ,ⅡandⅢbetween benign and malignant nodule (P>0.05). There was statistical significance in typeⅣ,Ⅴbetween benign and malignant nodule (P<0.05). There was statistical significance in "peripheral bronchiectasis sign" between benign and malignant nodule (P< 0.05).Conclusions:There was statistical significance in typeⅣandⅤand no statistical significance in typeⅠ,ⅡandⅢbetween benign and malignant nodule. The typeⅣis in a high probability in malignant SPN and typeⅤin benign SPN. The "peripheral bronchiectasis sign" is of the statistical significance in benign and malignant nodule and the probability of benign nodule is significantly higher than malignant nodule. The relativity between solitary pulmonary nodule (SPN) and bronchus, especially the "peripheral bronchiectasis sign", play an important role in the differential diagnosis of SPN.
