Objects To detect and explore the level of CK19 mRNA expression in rinse solution of axillary for breast cancer patients with mastoscopic axillary nodes clearance (MANC). To analyze the result of cytological examination in rinse solution of axillary for breast cancer patients with MANC. To provide reliable evidence for the safety of the operation.Methods1. The cytological examination in axillary rinse solution was done in 50 breast cancer patients with lymphatic metastases. Among these patients, there were 20 cases with MANC and 30 cases with general axillary nodes clearance (GANC). The relation between cytological examination and clinical parameters was analyzed.2. The special primers of CK19 mRNA were designed and selected, glucose3-phosphate dehydrogenase(G3PDH) as inner reference, And CK19 mRNA and G3PDH mRNA were amplificated by nested RT-PCR. Axillary Rinse Solution were collected in 50 patients with breast cancer including 20 cases with MANC and 30 cases with GANC. The amplified fragments were examined by 1.5% agarose gem electrophoresis. The IOD of CK19 mRNA were analyzed by gel analysis software Gelpro 4.0.Results1.The relation between cytological examination and clinical parameters:The positive rate of exfoliated tumor cells in patients with MANC and GANC groups was 25.0% and 30.0% respectively. (P>0.05) With the increase of the tumor stage,the disease progress and the increase in the number of metastatic lymph nodes, laparoscopic group shed the conventional group had higher positive rate of cancer cells, but the difference was not significant (P>0.05)There was no significant relation between positive rate of cancer cells and pathological types. (P>0.05).2. The expression levels of CK19 mRNA in patients with in MANC was significantly lower than in GANC. (P<0.01)Conclusion The expression levels of CK19 mRNA and the positive rate of exfoliated tumor cells in patients with in MANC was significantly lower than in GANC which indicated that there were less exfoliated tumor cells. The detection rate of exfoliated cell is related to the numbers of lymphatic metastases of the tumor.
