Objective:To investigate the relationship between genetic polymorphisms of COX-1-1676C>T and IL-1β-511C>T and upper gastrointestinal hemorrhage induced by enteric-coated aspirin in CHD patients.Methods:①400 cases came from Provincial Clinical College of Fujian Medical University,which included 100 CHD cases of taking aspirin for more than 10 days and the positive fecal occult blood test:ASP+OB(+)group,male59/female 41,average age 69.54±9.72; 100 CHD cases of taking aspirin for more than 10 days but OB(-):ASP+OB(-)group,male57/female 43, average age 68.68±9.06; 100OB (+)cases of upper gastrointestinal hemorrhage:OB (+) group, male51/female49, average age 67.62±13.31; 100 healthy volunteer:OB (-) group,male54/female46, average age 65.96±13.22. PCR-RFLP technology was used to detect 400 patients' genotype distribution (COX-1-1676C> T and IL-1β-511C> T).②Change of platelet aggregation were measured through turbidimetry among CHD patients.③IL-1βand VEGF levels are measured through taking ELISA in 42 CHD patients of two groups.Results:1. Genotype distribution of two genes are consistent with Hardy-Weinberg equilibrium (P> 0.05)in four groups. The differences of the clinical baseline data were not statistically significant (P> 0.05)in four groups.2. COX-1-1676 T allele frequency (68.5%) in ASP+OB (+) group were more than it in the other three groups, which were significantly different (P<0.05).IL-1β-511TT genotype frequency(ASP+OB (+) group was 46%, OB (+) group was 41%) and T allele frequency (ASP+OB (+) group was 69%, OB (+) group was 65%) as significantly higher than themof the twoOB (-) groups (P<0.05). The difference between ASP+OB (+) group and OB (+) group was not statistically significant (P> 0.05).3.After taking enteric-coated aspirin, platelet aggregation among CHD patients changed significantly, the differences is statistically significant (P< 0.05).4. After taking enteric-coated aspirin, the decrease of VEGF levels in ASP+OB (+) group was more than it in ASP+OB(-) group (29.02±11.30pg/ml vs 24.31±11.26pg/ml, P<0.05),.The rise of IL-1βlevels in ASP+OB (+) group was more than it in ASP+OB(-) group (9.26±1.41pg/ml vs 8.66±1.12pg/ml,P<0.05).5. The decrease of VEGF levels was compared in ASP+OB (+) group:IL-1β-511TT type(36.73±9.39pg/ml)and COX-1-1676 TT type (37.17±8.51pg/ml) was more significant than respective CC type and CT type(P<0.05).6.The rise of IL-1βwas compared in ASP+OB (+) group:COX-1-1676TT type(9.04±1.09pg/ml)and CT type (9.68±1.44pg/ml)was more significant than CC type(P<0.05). IL-1β-511TT type{(9.97±1.43pg/ml)was more significant than the other types(P<0.05).Conclusion:①COX-1-1676T allele may be an important factor of upper gastrointestinal hemorrhage induced enteric-coated aspirin.②IL-1β-511C> T mutation may be an dangerous factor of upper gastrointestinal hemorrhage.③The mutation of IL-1β-511C> T and COX-1-1676C> T may increase IL-1βlevels, which also decrease VEGF levels.It lead to upper gastrointestinal hemorrhage induced enteric-coated aspirin in CHD patients.
