Objective:This experiment established renal ischemia-reperfusion damage model, effect of different doses of remifentanil on renal histology and renal bcl-2, bax protein expression, and mechanism of its renal ischemia-reperfusion injury and the mechanism for the clinical anesthesia reference.Methods:Wistar rats were 48 male and female, regardless of (weight 230~280 g), were randomly divided into 4 groups (n=12):experimental control group (S), ischemia-reperfusion group (I/R), low-dose experiment group (L), high dose experimental group (H). Clamping the left renal vein, ischemic 1h, and then restore blood flow reperfusion 6h, renal ischemia-reperfusion rat model. S group, but not isolated left renal vein occlusion, L before ischemia 30min continuous femoral vein pump Ru Ruifen fentanyl 0.2μg·kg-1·min-1, H group sustained femoral vein 30min before ischemia pump Ru Ruifen fentanyl 1.0μg·kg-1 min-1; S group and the I/R groups received the same volume of saline.6h respectively reperfusion rats were killed, take the left renal upper pole in 10% neutral formaldehyde solution fixed, paraffin-embedded after dehydration, sections (thickness 4μm), line of HE staining, light microscopy of histopathologic changes in the structure, determined by immunohistochemical bcl-2, bax protein expression, TUNEL method for the determination of renal cell apoptosis.Results:Compared with S, I/R, L group and H group upregulation of Bcl-2, Bax protein expression decreased, Bcl-2/Bax increased degree of enhancement of renal injury, renal cell apoptosis index increased (P<0.01 or 0.05); and I/R group, L group and H group of Bcl-2 protein expression, Bax protein expression down to reduce kidney damage, renal cell apoptosis decreased (P<0.01); and L group, H group of Bcl-2 protein expression, Bax protein expression down to reduce kidney damage, renal cell apoptosis decreased (P<0.01 or 0.05).Conclusion:The rat femoral vein infusion of two doses of remifentanil can reduce the renal ischemia-reperfusion injury, which to 1.0μg. kg-1. min-1 continuous infusion is appropriate. Remifentanil to reduce renal ischemia-reperfusion injury in rats and increases Bcl-2 gene expression and reduced the expression of Bax gene.
