Partâ… MTT directing drug selection in intravesical chemotherapy for superficial bladder cancerObjective:The study investigated the correlation between MTT chemotherapy response assay and clinical outcomes after MTT-guided chemotherapy for superficial bladder cancer.Methods:From August 2006 to December 2007,Chemosensitivity of 52 superficial bladder cancer samples was studied with MTT assay, analysis the sensitivity rate of 12 anticancer drug in different pathological grade,stage and intravesical instillation according the results of drugs sensitivity test.The control group 58 cases were given intravesical instillation basing on the experience of doctors. Follow up all the patients and calculate the recurrence rate.The recurrence-free survival rate evaluated according to Kaplan-Meier.Analysis of gender,age,tumor grade and stage of recurrence risk between the two groups with COX proportional hazard model.Results:(1)In different individual patients,the inhibition rate of each drug was obviously discrepan.The sensitivity rate of cisplatin (DDP),paclitaxel (PTX), adriamycin (ADM),hydroxyl topotecan base (HCPT),epirubicin (EPI) higher, and methotrexate (MTX),vincristine(VCR),gemcitabine (Gemzar)showed resistance.The sequence of the highly sensitive rate is DDP>ADM>EPI>PTX.(2)In addition to MCC PYM,no correlation has been found between the phenotype of chemosensitivity assay and the pathological grade(P>0.05).(3)There was no correlation between the Inhibition of drug and the tumor staging.(4) The recurrence-free survival rate evaluated was significantly better in MTT group than in Control group(P=0.02)(5)The male patients which age>65,tumor size>3cm,G3,will tend to be more beneficial clinical effect with MTT-Guided chemotherapy.Conclusion:(1)The inhibition rate of each drug was different with individual patients.(2)According to the results of drug sensitivity test to choose the optimal chemotherapy drugs,which could increase the recurrence-free survival rate of two years obviously.(3)The male patients which age>65,tumor size>3cm,G3,we recommend chemotherapy guided by tumor chemosensitivity.Partâ…¡Adenosine triphosphate based tumor chemosensitivity assay (ATP- TCA)for individualized chemotherapy in patients with superficial bladder cancerObjective:To investigate the heterogeneity of chemosensitivity in superficial bladder cancer using ATP-TCA assay.Observed the differences Sensitive rate between single drugs and combination of drugs.Methods: From June 2009 to Januarly 2010,Chemosensitivity of 21 superficial bladder cancer samples which received TUR-Bt were enrolled in the present study.To evaluate the chemosensitivities of 5 anticancer drugs such asTHP,ADM,EPI,HCPT,MMC and Gemzar+DDP+PTX,MTX+VCR+EPI+DDP,5-Fu+DDP+THP using ATP-CRA assay.Results:(1)The inhibition rate of each drug was obviously differentIn individual patients.The sensitivity of five single drugs were THP (77.78%),EPI(72.22%),ADM (50.00%),HCPT(38.89%),MMC(11.11%).(2)The mean inhibiting rates of antitumor drugs in the primary group were notedly higher than that in the recurent group.(3)The inhibition rate of combined treatment is higher than single drug.(4)The short-term recurrence rate of three groups,ATP groups is lowest. Conclusion:(1)ATP-TCA assay for drug sensitivity test in vitro might provide experimental basis for clinical individual chemotherapy of superficial bladder cancer. (2) When patients need intravenous chemotherapy,ATP-TCA guided chemotherapy can serve as an important basis for drug selection.
