Comparative Study Of Using Conventional Method And Piezoelectric Osteotomy In Extraction Of Impacted Mandibular Third Molars

Objective:To compare intraoperative and postoperative complications.,time of surgery between with the traditional method and piezoelectric osteotomy in extraction of low site horizontal impacted mandibular third molars.Methods:From Dec 2009 to May 2010, in oral surgery clinic in Xiangya second hospital of CSU, Total 28 patients of impacted wisdom tooth of mandible, Which are divided into two groups by machine,15 cases are treated with the high-speed turbine and chiseling bone methord; 13 cases are treated with the piezoelectric osteotomy The operation is finished by experienced dentists. The patients need the futher consultion after 2 days and 7 days, to compare the time cost, intraoperative indisposition;local pain and swelling; compare limitation of mouth opening, numbness, dry socket, TMJ pain, infection between the two groups.Results:1.In extraction of lower horizontal mandible impacted third molar, no statistic difference showed in time cost, while significant difference showed in intraoperative indisposition. Patients in piezoelectric osteotomy group had less fear or pain(p<0.05).2.Comprising the results of futher consultation, There were significant differences between two groups in relation to pain, trismus and swelling.(p<0.05)3.In this research,there was no case with inferior alveolar nerve dysaesthesia, infection and dry socket in the group of piezoelectric osteotomy, however there were three cases with inferior alveolar nerve dysaesthesia in the group of traditional method.Conclusion:1.Piezoelectric osteotomy is better than traditional method to reduce pain, trismus,swelling and discomfort,avoid inferior alveolar nerve dysaesthesia, infection,dry socket etc. intraoperative and postoperative2.Piezoelectric osteotomy can cut bone exactly and reserve more heathy bone than traditional method.3.Piezoelectric osteotomy is diferent from the conventional techniques utilized, Piezoelectric osteotomy can be proved to be a safe and useful technique in surgery of extraction of lower horizontal impacted mandibular third molars.
