OBJECT:To understand the clinicopathological characteristics and the influencing factors associated with prognosis of patients with intrahepatic cholangiocarcinoma (ICC), and to provide valuable information for improving survival.METHODS:The clinicopathologic data of 92 patients with ICC who received surgery between January 2004 and January 2009 at the Second Xiangya Hospital of the Central South University (changsha, China) were reviewed, and their clinicopathological characteristics, and survival were analyzed.RESULTS:Included in all of the 92 ICC patients were 51 males and 41 females (ratio,1.2:1), ranging in age from 24 to 80 (54.2±11.63) years. Among the 92 ICC patients,33.7%(31/92) were assosiated with intrahepatic duct stones, and abdominal pain(84.7%) was the most common initial symptom. Laboratory tests showed,21.7%(20/92) patients were HBsAg positive,2.7%(2/75) had a-fetoprotein>30μg/L, 64.3%(18/28) carbohydrate antigen 19-9 (CA19-9)> 37 U/m,22.8% (21/92)had clinical jaundice,53.2%(49/92) patients were found tumor located in the left liver lobe,68.5%(63/92) got singal tumor, and 48.9% (45/92) received curative resection. Survival data was available for 81 (88.0%) patients and their median survival time was 11.5 months. the 1-,3-and 5-year survival rates was 48.1%,23.9%,14.1%,respectively. Multivariate analysis indicated that curative resection, the level of TBIL and number of tumor were the independent influencing factors associated with prognosis.CONCLUSION:ICC was difficult to diagnosis in the early stage. Curative resection, the level of TBIL and number of tumor were the independent influencing factors associated with prognosis, and surgical curative resection is still the most effective mean to cure ICC.
