Background: Human avian influenza is an emerging threat to communities worldwide. Since 2003, there had been 38 human cases, and 25 deaths in Chinese mainland. The development of qualified and able healthcare workers has became a new urgency in building the AI prevention and control system. Although Chinese goverment had carried out a series of training programs to improve preparedness capability of healthcare workers in rural area since the outbreak began in 2003, it remains unclear, to the author'knowledge, if these training programs are feasible and effective in improving the preparedness capability.The purpose of this study was to investigate knowledge about AI among healthcare workers in rural area.Objective: To understand the level of awareness of Human Infection of Highly Pathogenic Avian Influenza among health technical personnel in rural health agency and past training (2006 ~ 2007) and training requirements; to provide basis for conducting training by all levels of related departments.Methods: Through a lot of literature review and theoretical analysis, survey tools and implementation plan were developed. Then, experts gave some modification and adjustment to the survey tools. Based on this, group sampling method was used in questionnaire survey conducted among all technical personnel of rural health agency from 6 sample counties in Anhui and Liaoning province.Results: (1) The score(median/quartile range) in CDC was 55.0(39.0~67.0). Statistical significe was found between persons with different training experience (P<0.05). The awareness of persons in Acute Infectious Disease Department and Laboratory Department was 58.3% and 58.2% respectively. The score in medical establishment was 53.10(35.40~76.70). Statistical significe was found between persons with different training experience, training time, departments and titles (P<0.05). The awareness of persons in key office (breath internal medicine department) was 43.8%. The score in town health centers was 75.00(50.00~87.50). Statistical significe was found between persons with different training experience, training number and departments (P<0.05). The awareness of persons in key office was 70.4%. The score in town health centers was 100.00(88.80~100.00). The awareness of core information was 89.6%. (2) All health healthcare workers could acknowledge prevention responsibility. Radio and television media, anti-epidemic departments, the town health centers were very shortest period of convenient access to information channels.Founding:The survey found that past training had obvious effect on the awareness of human avian influenza among rural healthcare workers, and training time significantly was positively related to the level of awareness. Emergency drill as training contents and a method to improve the ability of emergency response was an important means to enhance the activity. All health healthcare workers could acknowledge prevention responsibility and the work procedure when they found unexplained pneumonia or human avian influenza cases.Suggestion: (1) make greater efforts in training, especially county-level health institutions; (2) the training textbook should be close to reality and need of rural healthcare workers to enhance its practicality and pertinence; (3) take measures to promote emergency drill to improve their response ability; (4) get the best out of the village doctors in the field of monitoring, report and technical relying on their good awareness.
