ObjectiveTo investigate the correlation between the imaging signs and serological index of HBV related HCC.MethodsTo collect 50 cases HBV related HCC of our department from Jan.2007 to Oct.2010; the diagonosis criterion of HBV related HCC accord with Management of Hepatocellular Carcinoma: An Update(AASLD 2010); to detect the serological index, such as the quantity of HBV M,HBV DNA,AFP,HA,ALT and so on; to do the imaging tests, such as B ultrosound,CT and MRI; Then do some statistical analysis about the correlation between the serological index and imaging signs of HBV related HCC.Results1. Of all the 50 cases of HBV related HCC, male cases account for about 88%,female cases account for about 12%, the average age is 50.73±9.94 year-old.2. Of all the 50 cases of HBV related HCC, 72% cases with the hepatitis B infection most likely acquired perinatally or in early childhood.3. Of all the 50 cases of HBV related HCC, the mean of AFP is 863.14±858.29μg/L, 74% cases were more than the upper limited normal(ULN).4. Of all the 50 cases of HBV related HCC, 92% of the tumor were in the right lobe of liver, 8% were in both sides of the liver, 0% was in the left lobe of liver alone.5. Of all the 50 cases of HBV related HCC, the HBcAb was positive in all cases; the mode of the quantity of HBsAg,HBeAb,HBcAb more than ULN (positive)were account for 80%(40 cases), the mode of HBsAg,HBcAb positive account for 12%(6 cases), the mode of HBsAg,HBeAg,HBcAb positive account for 6%(3 cases),the mode of HBsAg,HBeAg,HBcAb positive account for 2%(1 case). 6. Of all the 40 cases (case group)of HBsAg,HBeAb,HBcAb positive mode,HBV DNA in 82.5% cases was more than ULN(1000 copies/ml),66.7% of HBsAg,HBcAb positive cases were more than ULN in HBV DNA,100% of HBsAg,HBeAg,HBcAb cases were more than ULN in HBV DNA,1 HBeAb,HBcAb positive cases was less than ULN in HBV DNA.7. Of all the 40 cases of HBsAg,HBeAb,HBcAb positive mode,statistical analysis showed that HBeAb has positive correlation with HBV DNA(r=0.374,P=0.018).8. To select 45 cases of HBsAg,HBeAb,HBcAb positive CHB randomly,as the control group,15.6% cases in the control group were more than ULN in HBV DNA, compared with the 40 cases of case group in HBV related HCC, the difference was significant(χ2=38.093,P=0.000); HBeAb has no correlation with HBV DNA(r =0.084,P=0.324)in control group.9. Of all the 50 cases of HBV related HCC, the mean of ALT is 61.92±37.04U/L, statistical analysis showed that the HBV DNA has no correlation with ALT(P=0.722).10. Statistical analysis showed that AFP has positive correlation with the size of tumor(r=0.516,P=0.000), and so was HA(r=0.328,P=0.002).Conclusions1. Male cases,who were 40-60 year-old and with the hepatitis B infection most likely acquired perinatally or in early childhood, should monitor the HBV related HCC at a proper interval.2. The tumor was more likely occurred in the right lobe of liver,when do some imaging tests to monitor the tumor, be careful about the changes in right lobe of liver.3. The HBcAb can be detected in all HBV related HCC cases; the quantity of HBsAg,HBeAb,HBcAb more than ULN mode was the most common mode.4. The tumor were more likely occurred in the mode of HBsAg,HBeAb,HBcAb positive HBV related HCC cases which always with HBV DNA replication and HBeAb has positive correlation with HBV DNA(r=0.374,P=0.018),so we should be careful about this mode of HBV infection. 5. Statistical analysis showed that the HBV DNA has no correlation with ALT(P=0.722), at the same time, not all the HBV related HCC cases with the elevation of AFP(74%).6. AFP has positive correlation with the size of tumor, and so was HA.
