| Objectiveto investigate the trend of screening positive rate and correlative factors of Retinopathy of Prematurity ( R0P ) ,Based on the result,some beneficial intervention measures were proposed for the better prevention of it.MethodsTotally 1116 infants with birth weight <2000g in Guangdong women and children's hospital from 1 January 2005 to 31 December 2008 were enrolled in this study. The person who collected the clinical data arranged the order number according to the admission sequence for the infants and completed the questionary of ROP screening, The ROP examination were performed by professional ophthalmologists equipped with indirect ophthalmoscope. ROP severtity was defined according to international classification of ROP. Positive rate of ROP screening, proportion of screening,follow-up, different birth weight and gestational age, and critical patients within the four years were compared to study the trend of positive rate of ROP screening and correlative influence factors. Cochran-Armitage Trend Test and Correlations Spearman method were applied in the study.Results1.The positive rates of ROP screening increased gradually from 2005 to 2008, which were 5.8%, 8.5%, 13.2%, 14.5% respectively .The proportions of threshold ROP were 1.9%, 1.9%, 2.0%, 2.3% respectively, which had no statistical significance.2.The positive rate of ROP screening in infants with birth weight <1500g was 19.1%, 5.8% in those with birth weight between 1500g and 1999g, both had statistical significance by Correlation analysis (R=0.20,P<0.01); The positive rate of ROP screening in infants with gestational age <32w was 18.3% and 4.9% in those with gestational age≥32w, both had statistical significance by Correlation analysis (R=0.20,P<0.01); The positive rate of ROP screening in critical patients was 15.4% and 5.6% in ordinary patients, both had statistical significance by Correlation analysis (R=0.14, P<0.01).3. From 2005 to 2008 ,the proportions of screening respectively were 56.0%, 81.3%, 93.1%, 93.7%; The proportions of follow-up were 44.8%, 54.4%, 62.4%, 75.3%; Proportions of the infants with birth weight <1500g who had finished follow-up were 40.4%, 42.5%, 45.7%, 52.1%;Proportions of the infants with gestational age <32w who had finished follow-up were 42.3%, 50.0%, 54.3%, 57.3%;The proportion of critical patients who had finished follow-up were 51.9%, 60.4%, 67.5%, 71.4%; All were rising steadly within the four years and associated closely with the trend of positive rate of ROP screening.Conclusion1.The survey shows that the positive rate of ROP screening is ascending steadly within the four years,but the proportions of threshold ROP is not ascending obviously.2.The increasing positive rate of ROP screening might be associated with more proportion of very low birth weight infants, low gestational age and critical infants. Apart from that, more standard management of ROP also was a contributive factor.3.Effective intervention measures such as suggesting the basic hospital to transport the children at high risk to the hospital having qualification, paying more attention to oxygen and comprehensive prevention measures should be taken to reduce the incidence and blind rate of ROP. |
