Objective:During the peritoneal adhesion, it was existing in belly cavity of the process of the infection and inflammation, then the fibroblasts migration, proliferation and secretion collagen. To seek the sites of peritoneal adhesion prevention and treatment. Then this research choices subshrubby peony root-bark to discuss significance of prevention and cure peritoneal adhesion. At last, to investigate the effects of subshrubby peony toot-bark on the proliferation and apoptosis of adhesion fibroblasts, so that the mechanism which subshrubby peony toot-bark affect peritoneal adhesion can be disclose to provide the theory base for clinical prevention and treatment on peritoneal adhesion. Methods:From Dec 2009 to Nov 2010,2 cases of adhesive intestinal obstruction were chosen, both of whom underwent bowel resection and adhesiolysis, because of intestinal necrosis. Through optic microscope, pathomorphology of peritoneal adhesion was observed, with HE, V-G and Masson staining. To distinguish the structure feature of fibroblasts and collagen. After this, 60 healthy Wistar rats were randomly divided into three groups with twenty animals in each group. The surgeries on them were different among the different groups: Group A (Trauma +Infection group), intestines were clamped to establish an traumatic model, besides coliform were putted into peritoneal cavity; Group B(Trauma+ Infection+ Chinese herbs group), the procedures made were the same as those in Group A.Besides,after 12 hours subshrubby peony root-bar was perfused into stomaches 4ml per day; Group C(Normal control group), without any treatments. Rats were killed at 14th day, postoperatively in turn to observe their status of peritoneal adhesion. The adhesion tissues were taken to dye in HE. Peritoneal adhesion derived fibroblasts cultivated in vitro and were observed and analysed through light microscope, cell count, flow cytometry and Annexin V FITC/PI test. Then the effects of subshrubby peony toot-bark on biological behavior and apoptosis of peritoneal adhesion fibroblasts have been determined. Results:In peritoneal adhesion tissue, mesothelial cells and fibroblasts cells are primary cells, large amounts of interstitial collagen-based proliferation, in which a small amount of inflammatory cells can be seen, and the small blood vessels grow in. Mesothelial cells in the outer layer of adhesive tissue, in which there are a lot of collagen with free sequence alignment. The formation of fibers mixed with a large number of spindle cells. The degree of adhesion status decreased in the following turn:Group A >Group B> Group C. Its possible mechanism includes inhibiting fibroblasts proliferation or promoting fibroblasts apoptosis et al. In vitro, compared with the control group, the experimental subshrubby peony toot-bark group could be observed changes on inhibition of fibroblasts growth and proliferation, up-regulation of peritoneal adhesion fibroblasts apoptosis significantly. Conclusion: Fibroblasts and collagen-based extracellular matrix is the basis structures of adhesive tissue. Treatment of subshrubby peony root-bark can firmly decrease the occurrence and degree of peritoneal adhesion. Subshrubby peony toot-bark can suppress growth and proliferation of peritoneal adhesion fibroblasts and promote apoptosis. It indicates that, as a negative modulation factor, subshrubby peony toot-bark has its importance on treatment of peritoneal adhesion.
