The Characteristics Of Lymph Node Metastasis Of Inlet Of Thorax In Thoracic Esophageal Squamous Carcinoma

Objective:To understand the characteristics of lymph node metastasis of inlet of thorax in thoracic esophageal squamous carcinoma, to investigate the reasonable clearing range of upper mediastinal lymph node.Methods:The clinical data of 150 patients with thoracic esophageal carcinoma who had undergone radical surgery of esophageal carcinoma three-cut in affiliated tumor hospital of xinjiang medical university from Nov 2004 to Jun 2010 by univariate and multivariate methods. Resuts:lymph node metastasis of all patients was 60.67%, lymph node metastasis of inlet of thorax was 32.67%, metastases was found in 20.99%.thoracic esophageal squamous carcinoma(upper,middle,inferior segment)can metastasis to the lymph node of inlet of thorax, the lymph node metastasis of inlet of thorax of each segment(upper,iddle,inferior) was 57.69%,28.89%, 23.53%(χ2=9.02,P>=0.011);the lymph node metastasis of different differentiation(well, moderately,poorly) was 13.04%,40.91%,43.75%(χ2=11.665, P=0.003), the lymph node metastasis of tumor invation(Tl,T2,T3,T4)wasO,31.03%,34.58%,50%; the lymph node metastasis of different tumor length(<3cm,3-7cm,>7cm) was 11.11%,36.36%,31.82%; Usingχ2 test,we can see the metastasis of the upper thoracic esophageal squamous carcinoma and poorly differentiation of thoracic esophageal squamous carcinoma is the highest. Univariate analysis showed there was statistics difference in lymph node metastasis and metastases of inlet of thorax of different differentiation, There was not statistics difference in lymph node metastasis of the tumor invation and length.,but metastases was.Multivariate analysis showed that tumor histological differentiation and different segment was prognostic factor for lymph node metastasis of inlet of thorax.Conclusion:the dissection of the entrance hall lymph node metastasis should be taked into account;The dissection of lymph node of inlet of thorax had an important part in preventing localized recurring and metastasis of the postoperative of the postoperative of thoracic esophageal squamous carcinoma.
