Objective:To investigate the clinical effects of tuberculous empyema treated by four surgical methods, and to observe the changes of eosinophils in pleural fluid before and after treatment.Method:77 cases of patients with tuberculous empyema were treated by pleural decortication, muscle flap pedicle graft within the thoracic cavity, pleural thoracoplasty and lung resection according to their respective symptoms. The clinical efficacy of four surgical methods was observed postoperative. The changes of eosinophils in pleural fluid of each patient were detected before surgery and after 1 week,2 months.Results:1.56 cases of patients treated by pleural decortication fiberboard, the cure rate was 96.43%, and the adverse reaction rate was only 1.79%; 7 cases of patients treated by muscle flap pedicle graft within the thoracic cavity, the cure rate was 85.71%, no adverse reactions; 10 cases of patients treated by pleural thoracoplasty, the cure rate was 80.00%, no adverse reactions; 4 cases of patients treated by pleural pneumonectomy, the cure rate was only 50%, the adverse reaction rate was 25%.2. There was no significant difference in efficiency of fibreboard pleural decortication, muscle flap pedicle graft within the chest and thoracoplasty (P>0.05). Compared to other three operation methods, the effective of pleural pneumonectomy had a significant difference (P<0.05).3. The cure rate of 77 patients treated by surgery was 90.91%, and the total effective rate was 97.4%, with no deaths.73 cases were cured followed up for 6 months to 1 year, the cure rate was 94.81%,4 cases of empyema recurrence, the recurrence rate was 5.19%.4. The phenomenon of eosinophils increased was occurred in 24 patients within 77 cases of patients. It occurred in both surgical groups. The leves of eosinophils were significantly decreased 1 week after surgery (P<0.05), but still higher than normal; The leves of eosinophils dropped to below 10% 2 months after surgery, and there was a significant decrease than that of 1 week after surgery (P<0.05). There was no significant difference in surgical treatment groups (P>0.05). All patients were followed up for 6 months to 1 year without recurrence.
