Objective:To compare the incidence and intensity of Endodontic interappointment pain (EIAP) undergoing root canal preparation with different apical termination and to evaluate the influence of factors(gender. tooth type, previous periapical status) affecting the pain experience.Methods:82 posteriors from 82 patients requiring root canal therapy were included in this study. Patients were randomly divided into two groups. Group A (n=49) were prepared with apical termination "0.5" on ROOT ZX; group B (n=33) were prepared with apical termination "1.0" on ROOT ZX. The canals of all teeth were prepared using a preflaring (2/3 cervical-middle) and step-down preparation techniques. The related factors consisted of gender, tooth type, previous periapical status were recorded before treatment. Intensity of pain experienced was recorded on a visual analogue scale (VAS) of 0-10 within the periods of 48h after the initial treatment appointment. The data of incidence were statistically analyzed using the chi-square test.Results:The overall incidence of EIAP was 25.6%. The frequencies of EIAP were 36.7%(18/49) in group A and 9.1%(3/33) in group B. There were statistically significant differences between the groups (p<0.01). The EIAP incidence of absence periapical radiolucency significantly higher than presence periapical radiolucency in group A (P<0.05). Gender and tooth type did not influence significantly the incidence of EIAP (P>0.05), although females, mandibulars, molars had higher incidence of EIAP than males, maxillaries, premolars.Conclusions:The EIAP incidence was highly significantly associated with the apical termination of preparation. Root canal preparation with apical termination "1.0" on ROOT ZX can prevent or reduce the incidence of EIAP.
