The Analysis To Curative Effect Of Total Hip Arthroplasty For Treatment Of The Severe Developmental Dysplasia Of The Hip

Objective:To evaluate the effect and operation key Point of THA in treating to severe DDH.Methods:We treated 30 Patients diagnosed as CroweⅢ-Ⅳtype DDH with THA from January 2005 to November 2010 in the department of orthopadics.25 hip joints of 21 patients were followed up.There were 8 males and 13 females, the average age was 53.7±9.3Y, the youngest was 25 years old and the oldest was 69 years old.According to the classification of Crowe,there were 16 cases in typeⅢ,5 cases in typeⅣ.Nobody had an operation of acetabular osteotomy.All Patients had advanced osteoarthritis Performance.At the Acetabular side,all hips were used the biological-type prosthesis.At the femur side 14 hips were used the biological-type and 11 hips were replaced the cement-type. All acetabular prosthesis were implanted in the original location.16 hips acetabular restructed by autogenous femoral head structural grafting.The results use the modified Harris score;SF-36 table; X-ray imaging to assess.Results:No postoperative infection or serious complications such as nerve damage.Harris score improved from average 32.3±8.1 preoperative to average 85.3±11.9 postoperative.Every dimensions of SF-36 score also improved.No postoperative prosthesis position mal-Position.no cases of prosthesis loosening.Imaging display:metal acetabular cup are at position;femoral prosthesis without sinking.16 hips of restruced acetabular cup healing well without obvious absorption collapse. Preoperative and postoperative Harris score improved with levels of Crowe typing into a negative correlation.Total hip arthroplasty for severe hip dysplasia have a good effcet to resolve the pain.The patients are mostly moderate or more pain before surgery.Most patients can get a pain-free joints.Conclusion:The THA is the best option of the treatment of DDH especially with advanced osteoarthritis.lt relief and restore joint movement and joint stability aspect very well.If you want to master this very difficulty operation, it was a long learning cycle.You must accumulate sufficient experience and knowledge and do well prepare in Pre-operative design, conceived intraoperative risk and how to handled these dangers.
