Objective To investigate the expression of SARA in the kidney and observe the expression of SARA and analysis the ralationship of SARA and tubulointerstitial fibrosis in diabetic kidney disease patients combined with clinical index.Methods 35 patients were enrolled into the study, they were diagnosed as DKD (n=15) and minimal changes disease (primary kidney disease, control group, n=15) based on clinical manifestations and renal pathology and normal controls (n=5),Renal pathological changes were observed by HE, Masson and PASM staining. Interstitial damage score and collagen deposition score were evaluated. The expression of SARA was tested by immunohistochemistry and the relationship of SARA and tubular interstitial damage in DKD was analyzed combined with clinical indexes.Results HE, Masson and PASM staining showed Varying degrees of renal impairment of DN patients such as moderate to severe proliferation of mesangial matrix, nodular sclerosis, diffuse or segmental thickening of the basement membrane, tubular atrophy and compensatory expansion, local interstitial fibrosis. Meanwhile, the lesions in the kidneys of minimal change disease patients and normal controls were less severe. Interstitial damage score and collagen deposition score in the kidneys of DKD patients were higher than that in control groups. Immunohistochemistry showed that the expression of SARA was significantly down-regulated in DN patients compared with that in control groups. Blood glucose in diabetic nephropathy group was significantly higher than the other 2 groups (ρ<0.05), and the serum creatinine in DN group was slightly higher than the normal control group and non-DN minimal change group, but no significant difference (ρ> 0.05). the diabetic kidney disease group and the normal controls, and non-DN group and normal controls show significant differences in the proteinuria level (ρ<0.05), and diabetic nephropathy minimal change group and non-DKE group had no significant proteinuria levels Differences (ρ>0.05).Conclusion This study of the expression of SARA in the renal tissues of DN patients, confirmed that the expression of SARA is negatively related with renal interstitial fibrosis and renal tubular damage; while tubular function is closely related to the glomerular Hardening ratio and the renal function,the results of this study suggest that renal tubular damage in diabetic nephropathy is glomerular damage aggravated factors, and SARA may become one of the diabetic kidney disease tubulointerstitial fibrosis therapeutic targets.
